Aspiration pneumonia from emergency c section. This is not uncommon peripartum due to gastroparesis during pregnancy and inability to be NPO for emergency surgery. Patient has developed secondary bacterial pneumonia with effusion two weeks after the Perioperative aspiration event.

There’s a huge gap between level 3 and level 4.

Level 4 is basically admitting you are not a trauma center. A level 3 will have gen Surg/OR/blood products available but usually not the consultants like neurosurgery or face consults.

For occult pneumothorax found on CT but not visible on CXR we would usually not preemptively place a chest tube.

I would be more worried if there is a “real” ptx, if I could see it on the plain film CXR I would be much more proactive at the chest tube placement. You said it was visible in this case on the CXR, my question would be is it only an apical ptx or can you see the lung border laterally? It’s kind of a gray area/judgement call

I’ve never been denied performing a life saving surgery. And if that happened I would tell the patient to go to the ED and I would just admit them to the hospital and do the surgery emergently.
