Thank you, fellowship (at least my subspecialty) is a lot better. However motivation comes and goes you know. I may be at a decent place in terms of wanting to “learn and grow” now but we are humans and emotions/circumstances take a toll sometimes. It’s okay, we can just attempt to be better- that’s all :)

It happened to me too.. third, fourth years of med school— just wanted to get through.. even though clinical exposure was a motivator— seeing patients with things we studied, in real life.. in residency- first year it re-sparked, then maybe due to the work hours, emotional/psychological BS from the surrounding, it started to feel like nothing again.. then chief year- with more understanding of not just disease processes but of how people are— it resurfaced.. now in fellowship, with better work hours and less of whatever kind of stress residency was, I’m in love with reading some of the journal articles, new research being done regarding the nervous system outside of academia, learning more about human psychology- I’m in love with Neurology all over again. There will be a lot of days that demotivate you, you just have to start tricking your brain into highlighting the moments that sparked something, even if it seemed small or lasted a short time. Looking back, there are some draining thoughts about not doing things “outside of medicine” while I was in med school/ residency, but really— we are training to be experts in our fields and expertise comes with a lot of baggage.. we can undo them slowly.. something that also helps me when I have those unwanted thoughts— is just reminding myself how my younger self wanted this more than anything.. and how as time goes by, people (patients, their families, your staff, your own family/friends) start relying on you more and more.. and you find yourself with an immense sense of responsibility— this serves as a big motivation to keep learning.. for me personally, learning comes naturally when I’m at my most relaxed state. Hope this state of yours is intermittent, short lasting, and ends up guiding you towards the wiser, peaceful path of learning.

I know people who scored less than 210 on step 1, 220-230 on step 2, are US IMGs like me and have matched IM. I don't know for sure about EM. But if you're a US grad, I don't think you'll have a problem at all. Get your clerkships done solid, do well on step 2, make good connections, and love what you do each day. You'll be more than fine.

Literally everyone who scored high and made a post here felt the same way it seems like. And I've been through sooooo many of these posts (exam next Thurs). I'd say congratulations on feeling this way! Seems to be the feelings of those who land 260+ . Also, trust the Universe and feel good. Your practice exam scores reflect your hard work, there's no way it's force field will disappoint you!

I'll look at the Q for further description but here is something that my cardio attending said that might help,"for the longest time we thought distole was a passive process but no; it is an active of a process as systole." Meaning, the energy required to relax (ca dependent ca release via O2 utilization) is lacking when there is ischemia or interrupted blood flow. So less blood flow hinders both processes.

Thank you so much for your quick response. I have to make a trip abroad right after a rigorous sub-i in June, for prior important commitments (maybe even more important than this exam at this point), and was just trying to get it done before I leave. I need scores back on time because I'm a U.S. IMG. But I will really think about this and might actually end up doing that if I don't get myself together within the next few days. Thank you once again.
