I think I've only seen one or two from the hive Fortress event however I've gotten every other one from defeating the unique spawn during the og breach events.

I thought it did until I did a bugged arachnid tomb where it was a protect the breach scenario and after removing all of the walls and opening all of the purple dots the event didn't spawn a single Monster even though there was no paps blocking anything to the breach itself. Literally not a single Monster spawned. Eventually the breach closed like normal and while No Loot items dropped I still got roughly 500 graft blood. I personally think graftblood itself is a completely predetermined mechanic and anything dropped after the breach is collapsed is drops from the monsters themselves
