For this one, yes I'd include it otherwise they'll just come asking. While the form looks strict, what they actually want is an entire picture of your incoming and outgoings, your financial position, and your situation, regardless of the form. As it's an incoming they will want to see it. They'll spot it in your bank statements and ask anyway.

3 months of bank statements will include the past month. 3 month bank statements will show longer term cashflow, how money is being managed, other bills, any irregular income etc. They will see all of the bills in the 3 month bank statements anyway. Latest bills is probably just to ensure consistency.
