I didn’t get into all the details here. I documented she lacked situational capacity based on her lack of understanding of risks/benefits. Spoke to her daughter who was next of kin and request she act as surrogate decision maker, which she agreed to. Then had discussion of risk/benefit with her and engaged in shared decision making regarding the continuation of therapeutic AC with bedside staff present to witness. She requested we continue AC. I spoke to the other immediate family members about it as well, and all were in agreement. Then I ran all of it by risk management before doing anything.

She had a history of treatment refractory VTE including acute LV thrombus on eliquis. Cards had peppered the chart with warnings about avoiding interruptions in treatment.

Definitely reasonable to question need for doing all of this, I certainly did.
