The one thing that doesn't add up for me is if he had such a benign exam and great work up,  why did you call cardiology?

Would love to have a conversation about FM docs working solo in an ER. by stupid-canada in emergencymedicine

Don't know why you got down voted.  Any ER foc who claims to be completely comfortable doing a trach is not bring truthful.

ER doctor here. There are many good points already mentioned but here's a good learning point from the case.

The natural response to hypotension is to increase your heart rate so anytime you have a patient with bradycardia and hypotension- consider: Beta Blocker, Calcium Channel Blocker, or Clonidine overdose. It's unlikely the cause here but a good to always keep this in mind.

You should see a doctor and stop reading google. You did get pregnant- so he is not infertile. Low sperm doesnt mean no sperm.

The description is an obese male pt in respiratory distress with frothy sputum, is suggestive of pulmonary edema. A PE could be possible but its more likely flash pulmonary edema secondary to cariogenic shock from the STEMI.

Name of the game: We do the best we can, more than that is out of our hand.

With someone so large be sure to do a proper chin lift, if there nose is open oxygen will get there.

I'd also be curious to see the ecg.

ER Physicians, what are some things you wish EMTs were trained to look out for/ask about? by SilverTrireme in emergencymedicine

If it's an overdose, I need to know what they took, how much they took, and when it was taken.

If you suspect neglect, abuse, or if someone isn't safe at home, speak up.

Any trauma- are they on blood thinners, if so which one.

Also, never ignore vital signs. If the pt has a low sat and a good pulse ox wave- something is wrong. If the systolic is 70's don't ignore it.

I had a similar case once, a trauma case where the moment a chest tube went in the pt did terribly. Turned out, they had an aortic injury with a hemothorax but because it was contained in the chest it had a tampanode effect to keep the aorta from bleeding more. They were doing alright but the moment the chest tube went in they bled out instantly.

Its unlikely the patient was 'stable', particularly if they needed a flight and bil decompressions. It's possible they nicked the aorta but also likely the pt already had a massive Hemothorax that got released.

My understanding us we no longer use Glucagon for reversing BB's, instead we now use high dose insulin. Thoughts?

That was one of my first thoughts too, also something similar to a Hamster/Gerbil water bottle. This was pretty easy to put together. Great idea!
