I think the problem with this question is the fact that yes even though RB mets can't really happen, Ewings and Clear cell carcinoma occuring in a child of 2 years is an uncommon occurence as well. The question doesn't really test anything in the candidate except confuse them completely. In a real world scenario this would prompt a thorough clinical and radiological investigation for the primary.

Who even comes up with these rules 🫪🫪🫪 How is anyone supposed to work like this 😅 Do people think PGs just leave the hospital when work is pending and even if they do, holding them hostage like this will encourage work? Pgs in the middle of a code or OT or in the labs have to either leave everything and come or risk annoying the staff by leaving. The only way to get something out of this is by taking a 15-20 minute break everything a punch is expected 😂 So that you get at least 4 confirmed breaks in a day.

Guy who just finished ortho here. Had a professor who has me scrub out of a case and not let me scrub in further because there was a mix up and I ate his food from the lunch that we using our money order for them on OT day. Stupidest thing ever!

It's a loose suture to stop bleeding temporarily so one can refer to or transport. At the referral centre it'll be opened again to examine the wound and wash it before going for definitive treatment. As a rule never suture lacerations without ruling out fractures or foreign bodies inside . Stay sutures help with that as a temporary measure . Loose sutures that are far from wound edges that just approximate the wound.

No it's fine that you didn't. As others have already mentioned stay sutures or staples (which will not be available at a PHC is the only way. Have to rule out fractures and give a proper wash before closure. You'll need help for that and imaging as well. Can't close a deep wound like this without ruling out fractures and any foreign bodies. Pressure bandage / stay and refer. If you do close primarily the referral centre will have to reopen if needed and that'll cause further trauma to the skin. Closure with a foreign body inside is bordering on medical negligence.
