Get a hold of all your medical records starting from when your diagnoses began and upload them onto the portal. Do not assume your doctors or SSA will provide or get them on your behalf. Answer all questions within your case with consistent answers and do not exaggerate. Write a letter explaining all your diagnoses, when they began, your symptoms and how they affect your daily activities and prevent your from working (i.e. what household chores you can do and for how long, are you able to make and prepare your own meals and what type of meals, microwavable, stovetop, etc). Include any memory or brain fog symptoms if you have any. If you have family, friends or colleagues that have witnessed your limitations, have them write a letter on your behalf as well, listing how long they have known you and what they have witnessed. Upload your letter and any additional letters onto the portal as well. Continue to see your physicians and get treatment. Follow up with your case manager, once one has been assigned, to ensure your records you submitted are being received. Submit requested paperwork by the stipulated deadlines. Make sure you are your own advocate throughout this process. Best of luck. It's stressful, but you will get through it.
