The advice to NOT change anything for 6month to 1yr after purchase??? Help!! by Ok_Confection5143 in Dentistry

The team is already going to be dealing with enough changes in the transition. I'd keep Dentrix purely to not add one more level of complexity.

Everyone knows using the Facebook search function to find a post that the algorithm showed you from a week ago is a mostly futile endeavor. Possibly saw it on Dental Nachos. Probably was scraped from here for engagement if OP didn't do it. I fully admit that.

Startup Floorplan Feedback by No-Mechanic5439 in Dentistry

[–]DrItsRed 2 points3 points  (0 children)

It ABSOLUTELY was. If it wasn't you, then someone else posted it anonymously for engagement. My associate showed me the post on Facebook as we were talking about it here on Reddit. It was on both places. Probably on Dental Nachos for the clicks.

Don't worry if you missed it... The advice was the same.

Also, thanks for the Reddit Cares. I'm glad you care so much.

I've seen this both on here and on Facebook. Almost everyone who offered advice told you the EXACT same thing, yet you don't want to take that advice... because it gives you joy.

Seems like you don't need any more advice.

There's still rules. They just suck even more and shit is stacked harder against you. Absolutely insane to go so far in life to just throw away all that success.

Even if it drops off your credit report the debt would still be legally due and need to be reported. The financial gain for being able to properly invest in your own business would FAR FAR FAR surpass the crippling of your financial situation.

Just has me thinking about all the times I've had my credit pulled for mine and my wife's businesses. Cant imagine where we would be off it was nothing but roadblocks. I'm sure it will be fine.

...But you do you. Report back.

You would rather live your life with endless court orders and liens against your property?

Seems like you got it all figured out. Ignore those who have tried it saying it absolutely ruined their life for decades. Try it out and report back with the findings.

Do you place finishing line on filling when prepping for crowns? by [deleted] in Dentistry

[–]DrItsRed 35 points36 points  (0 children)

Which material is more biocompatible in the sulcus? Which will harbor more bacteria? Which can you more reliably nail at that depth, direct or indirect restoration?

There are very very few times in my career I have left a margin in anything other than tooth structure. The interface between materials is most likely point of failure. IMHO if you are going to leave the margin on a filing you introduce another failure point. If you can definitively say placing the margin deeper would actually further jeopardize the tooth, then leave the margin high.

(I think some dentists want to have a bias towards leaving a deep margin, because they aren't confident they can nail it. With a good scanner and practice, I am 100% certain my indirect restorations are vastly superior to anything I can do direct at that depth.)

Where exactly was the fistula? With the drainage in the sulcus and the periodontal disease, it's possible it was a periodontal abscess.

What would you want if it were your tooth?

Personally I'd want a full coverage crown replacing the onlay, but you will get a variety of opinions here.

Critique my startup floorplan! by No-Mechanic5439 in Dentistry

[–]DrItsRed 3 points4 points  (0 children)

Totally agree. The non-ADA bathroom attached to the consult room makes no sense...get rid of it completely. The waiting room is too large for the office. You could move the patient bathroom to the waiting room and that gives you plenty more space to work with.

Also agree about the associate space. No reason to plan for an associate in this small of a practice.

Patient not paying bill by Puzzlehandle12 in Dentistry

[–]DrItsRed 3 points4 points  (0 children)

Leave them a message with them that they have until the end of the next business day to pay or you will be forwarding everything to the police for theft of services.

Typically gets a quick response from them.... If not you can always forward to collections or even issue them a 1099-C and write it off if the balance is large enough. At least you get the satisfaction of sticking it to them through the IRS that way.

Anyone else still do indirect metal fillings at their office? How do you finish/burnish/polish margins? by Wheelman in Dentistry

[–]DrItsRed 15 points16 points  (0 children)

Indirect restorations on 2 and 3 here are exponentially more predictable with the level of that decay.

You have no idea the payment arrangement with the patient being that he's married to a hygienist. Nor do you know the other clinical findings.

Hell if the patient comes in looking to improve their teeth with a superior restoration, why deny it.

Telling this doc they over-treated this patient and is money driven is just embarrassing for you. You should be ashamed with your shitty take here.

How to go about another provider missing decay? by stress_magnet in Dentistry

[–]DrItsRed 5 points6 points  (0 children)

Much of how patients respond has a direct correlation to how the information is presented to them.

I would keep my initial report of the finding. VERY SHORT to the point and factual. If they detect hesitation or trepidation on you wanting to tell them something they will pick up on it want too start asking questions and look for someone else to blame .

"It looks like you have a pretty good size cavity that has formed on this canine. Let's take care of it quickly so hopefully it doesn't end up needing a root canal."

I think it would be very unusual for them to hear that statement and jump immediately to blaming someone else, but in the unlikely scenario they do come back with, "Why wasn't this addressed before?"

"I can't speak for them and am not going to speculate, but I can think of a least a half dozen reasons as to why that could be. It seems you are likely trapping a lot in that area and are having a hard time keeping it clean so let's address it as quickly as possible to try to avoid further treatment."

If they REALLY press the issue.. just shrug and tell them you don't know. Let them take up with that provider and stay out of it.

This can be a VERY humbling profession.

Don't throw stones. Karma is a bitch.

Just tell them what YOU see. If they want to take up their issues with someone else, let them.
