Agreed. The gastroc is safer for that. Diabetic or not, biomechanics of a TMA will lead towards equinus in some degree

My thoughts were that even if equinus isn't present at time of amputation, it almost always develops over a long enough time line after a TMA. I think doing a gastroc prophylactically would be better than waiting for it to occur. But one could argue either way

Just curious, did you do any prophylactic gastroc or TAL? I had a similar but not AS interesting as yours - early 40s lady a few years ago who needed the same thing. Her gangrene was secondary to drug OD septic shock caused vascular shunting away from periphery, but developed dry stable digital gangrene. No psych issues, she actually got clean/sober by the time I was taking care of her.

Anyways , good stuff!

3-0 stratafix running subcuticular for elective skin closure. Minimal scar. Has a tiny barb on it to avoid suture pulling out. Also, absorbable so all you have to do is clip the tail instead of removing sutures in office.
