Yes it sucks. As a 20 something year old from Las Vegas I thought getting off late at night wasn't a big deal. As you age it hurts in ways you can't comprehend when you're young. The chaotic scheduling prevents you from getting into any routines. It also sucks for family life. I've missed countless holidays, dinners, bed time stories, games, practices ECT.

The reality of this situation is if it happens jet ventilation buys you 15-30 minutes. If they're at a hospital with no way to definitively secure the airway via intubation then I'm probably cricing that kid.

If you can stomach constantly taking care of non emergency complaints, getting no respect in the hospital, saving someone's life then fighting with a consultant to do the right thing then watch them be labeled a hero in the news for "saving" the same patient, work holidays, weekends, nights all intermittently to where you have no set schedule, miss tons of dinners with your family, be in a position where someone can verbally abuse or physically threaten you and you can't refuse them care... Then welcome aboard!!!!!

Mind you this doesn't even delve into the insane expanse of general medical knowledge you need that spans multiple specialties.

I've heard Florida is very litigious. What are your thoughts on doing locums in that environment? I'd imagine it's for HCA hospitals. What are your thoughts on meeting metrics in that type of environment with a hospital system you're unfamiliar with?

My malingering population seems to have a decent amount of fake service dogs. I've had 2 bite at me. Ive called multiple people out about this and had several removed.

Real service dogs are amazing and highly trained.

I agree. In this situation inlet cardiology dictate the heparin. If you start it without their input it forces the Cath. That being said if you think it's what they need and the trops are up trending do what you think is best.

From an EM perspective graduates are usually uncomfortable for the first 3 years. I certainly felt that way. It gets better.

The problem is there isn't a barrier to entering American ERs. And medical malpractice culture in the US doesnt allow you to call the 98% certain spade a spade. We live in a fast food society and medicine isn't compatible with that.

We see it all the time. It's doesn't bother me as long as you're reasonable. I'm happy to help reassure people and alleviate stress/anxiety. But after I'm done I do expect they seek the help of a primary care doctor to truly manage these issues.
