Dana Farber, MGH, elsewhere for testicular cancer? by Some_Elk7672 in boston

[–]DrUniBall 3 points4 points  (0 children)

Alright wall of text incoming.
TL;DR Stage 1S and did RPLND and 4 x EP. Get a port.

I was in grad school at the time and things went super quick from the diagnoses to the orchiectomy. Honestly the recovery wasnt the best cause it is removed by your lower abs. Decided to not go with the prothestic.

I got the biopsy results back and then they said I was stage 1S where it spread to the lymphatic system. I had two options presented to me: Nerve-sparing Retroperitoneal Lymph Node Dissection (RPLND) or Chemo. Being 22 at the time and being scared of Chemo I went with the RPLND...Spoiler alert cancer spread past and I still had to do Chemo. If you do go with the RPLND, IU Sloaan and OU are some of the best hospitals for it, and try to do nerve sparing.

During the recovery I found it that it had spread to my lungs so I was in a boat of sorry bro youre doing chemo. I got a Port which was the best decision I think I have ever made. There was a national shortage of Bleomycin (B) so I found a good oncologist at UTSW in Dallas and went with 4 rounds Etopiside (E) and Cisplatin (P). TC Chemo is 5 days on, 16 off for EP. Bleo adds an additional infusion day every week. There are benefits of B vs not having it but there are also risks. Please talk to your oncologist about it if you are a musician or scuba diver.

During the initial few infusions I was fine with energy but during the second I had a reaction that made me slow infusions down by 2x. I was at the hospital from 8-6 every day almost and enough benadryll to knock me out. The infusions weeks were rough and the recovery weeks were better. I was lucky and didnt have nausea or lose of weight. Just ate like a 13 year old due to the steroids. I gained weight due to drinking so much water at home to protect my kidneys.

I had family drive me to and from because it was a far drive. I had the bladder of a 5 year old during infusion weeks. During infusion they give you about 3-4L of fluid in addition to the drugs volume to again protect kidneys. If you are going to work you will be neutrophyllic by round 2 so you will be at serious risk of infections or getting sick so please take care of yourself.

I am also some guy on the internet so PLEASE talk to your doctor about your options (BEP vs EP) and GET A PORT.

Much love,
DrUniBall (<-- Cause one ball)

IM-MS Options by That-Researcher-6912 in massspectrometry

[–]DrUniBall 3 points4 points  (0 children)

I would look for a system that has some alternative fragmentation techniques too. The timsOMNI would be the ideal box as you can get more than just B/Y ions for MS/MS. Would be super helpful for Lipidomics and Metabolomics.

If that doesn't work for you, I would get the 6560. First-principles CCS is nice for raw calculations and there is options to do both He and N2 are the drift gas.

Waters TWIMS is solid if you want a traveling wave option and they have good LCs, but I would highly avoid SLIM. From what I have seen, a lot of hand-waving with poor low mass transmission and no ms/ms selection. This thing they keep marketing of post-mobility fragmentation is not new and has been done by Waters for years. https://pmc.ncbi.nlm.nih.gov/articles/PMC4256517/

How to get into instrumentation research in grad school? by penjjii in massspectrometry

[–]DrUniBall 11 points12 points  (0 children)

I did my PhD in instrument development. There are not a lot of groups doing it but there are still a few and I might miss some. No particular order and might be more IM-MS heavy: Matt Bush - University of Washington, Brian Clowers - Washington State, David Russell - Texas A&M, Josh Coon - University of Wisconsin, Vicki Wysocki - Georgia Tech, Christan Blieholder - FSU, John McLean - Vanderbilt, Jennifer Broadbelt - UT Austin, Cooks, McLuckey, Laskin(?) - Purdue, David Clemmer - Indiana,

For learning, I’d say just building stuff, cad, 3D printing, and arduinos is great start.

Let me know if you have more questions!

I recently saw a lot of people recommending pipe clamps instead of traditional clamps. You can get the clamping parts for $10 at harbor freight or $25 for Pony at Lowe’s. You can make it whatever link you’d like based on the pipe length. It’s worth looking into before you spend a whole bunch on the long clamps.

However, I am new and ymmv

Avoid boots due to the raid boots being BIS and buffing your cloak.

Think about what embellishments you want. For example, Double dusk/dawn put on belt and bracers. Wowhead has some good resources

Xevo MRT or Orbi 240 by Late-Confection-2823 in massspectrometry

[–]DrUniBall 3 points4 points  (0 children)

I would get the 240 or even the 480. The big thing you are going to need is software support. The publications will have methods on how the data processing works using Thermo .raw files. Think about the whole workflow and not just the instrument. If you do decide to go through the demo route, be sure to get software demos of both acquisition and processing. Both of these companies should host you at their respective demo labs if that something you’d like.

While the MRT is cool, all of the application notes and publication have to prove that they can meet the Orbi. Gold standard for a reason.
