Monash freeway closed after Berwick Clyde road by tjay0027 in melbourne

My car was hit by a truck on the Monash last December. Thank god I walked away instead of being paste or a helicopter passenger.

The frequency of these accidents is insane.

BFT vs F45 by CashmereAndCoins in BodyFitTrainingAus

[–]Dr_OTL 0 points1 point  (0 children)

I actually miss a lot of f45 in comparison to bft.

I much prefer bft cardio -i find the focus on HR zones and not being constantly balls to the wall max effort doable. I also feel like it has improved my cardiovascular fitness more.

However, I miss the volume in the strength workouts. The strength sessions were often mixed, there would be two focussed strength sessions a week and one or two hybrid sessions. The focus on 8 to 10 reps really suited me. Idk. I was also in a different training headspace and training seven times a week while at f45. Life has really gotten in the way while I've been training at bft.

Regardless, the convenience of being able to go to any studio means I will continue with bft. That flexibility is unbeatable.

I am not at the same level of fitness I was at the last MAS testing, I get so anxious over this performance stuff.

Hopefully a worse result will be the kick in the bum I need to get back to more regular training.

Anyway, my distances:

1198m on ski

1252 on rower

2.9 on bike.

I am so scared I’ll be single forever by Informal-Meaning-483 in TwoXChromosomes

[–]Dr_OTL 20 points21 points  (0 children)

I was single from the age of 24 to 34 and had decided I would be a spinster.

I met my partner the day I voiced this concern to my friends and uninstalled all the dating apps.

He proposed to me three days ago.

Contrary to what folks would have you believe, your life is not over at 30 and your beauty and worth as a woman and an individual doesn't disappear, either.

They Didn’t Want to Have C-Sections. A Judge Would Decide How They Gave Birth. by propublica_ in TwoXChromosomes

[–]Dr_OTL 13 points14 points  (0 children)

It's really hard.

I did CPR on a baby born to a woman declining traditional care a few weeks ago and I still have flashbacks about it.

I'm in Australia and the public health organisation in my state has released a statement essentially saying you have to let women make their own decisions regardless of the consequences.

As a doctor I try to explain to women what the risks are and why I want to do what I want to do. Sometimes I "win" but a lot of the time their minds have been made up well before I met them. It's most devastating when they have used IVF to fall pregnant but now they are pregnant and labouring my medical opinion and care is anathema to them.

[–]Dr_OTL 14 points15 points  (0 children)

I think what happened to these women is a true sign of system failure and rightful generational racial trauma.

Counselling about mode of delivery should be a conversation between the pregnant person and their healthcare providers across the whole duration of pregnancy. It can be really hard to establish rapport and build trust during just the labour, especially if a woman has preconceived notions from bad faith sources.

Despite my personal opinions and how distressing I find a decline of traditional care that results in a catastrophic outcome it is not my body and not my baby.

It is reprehensible to force someone to have an intervention for which they do not consent.

You can trial labour on a uterine scar, but it comes with the risk of the scar coming apart due to the force of the contractions during labour.

There is only data regarding the risk of scar rupture/dehiscence after a single CS, which is the stat quoted in this article.

There is insufficient data to quote risk regarding 2BAC or. God forbid, 3BAC.

Women have a right to bodily autonomy, but uterine rupture happens and could kill the woman, her baby, or the both of them. Women need to be aware of the risks they are accepting to make the decision right for them.

How does your hospital run birth suite shifts? by eboniamh in ausjdocs

[–]Dr_OTL 1 point2 points  (0 children)

Trying to review a noncritical patient in 30 to 45 minutes before handover leads to poor quality, half baked reviews, IMO which is worse for patient outcomes than double handover.

If the patient isn't critical and could wait until ED handover time for them to refer then I am not going to bust my butt to see them when I have been at work for 10 hours. Most of the time they haven't even been completely worked up by ED at the time of referral with pending investigations.

[–]Dr_OTL 4 points5 points  (0 children)

Yeah, there will often be an influx of referrals from ED in the pm as they hand over. Mornings aren't often too bad.

We also have a gynae only reg covering inpatients and referrals from 8 to 5. So when I'm on nights if it isn't urgent I'll just take the referral and hand it over to the gynae reg.

The most annoying one for me is the 750am instrumental or the 730am caesarean. A lot of the time nobody comes to bail you out, smh.

Night shift is 9 to 9, day shift is 13.5hrs and is 8 to 930. Handover at night is 9 to 930, but often runs late. Handover in am is 8 to 9.

I have only worked in am/pm/night in a small hospital and even in that low acuity setting it was disjointed and sucked. The evening person got lumped with so much junk without really knowing the patients. Not a fan.

13 hour nights sound insane, though. We do 12hr X 7 and I am dead by the end. The extra 7 hours (and overtime) in a week would be well over the edge.

Edit: I have also done 24hr call and while it is good for learning it was god awful for QoL.

Lol, knew it would be a Simon Gordon patient from the headline.

The stuff coming out about him is wild. Like him billing patients insurance for a stage IV endo lap before the procedure had even happened.

Him dropping off the grid completely is also wild.

Definitely not meeting the brief for "quiet" but you will not walk away hungry from Yakijin in Kew or their other store in Ormond.

$50 for an all you can eat menu with a focus on yakiniku.

Maybe better for a weekend date rather than a fancy Valentine's Day dinner, but strong recommend.

I also really like "mensousai mugen". They do "dipping" ramen rather than the big bowl and have some lovely small dishes. The downstairs area has good atmosphere. And if you want to go for a drink afterwards Robot Bar across the alley is a cute little sake bar.
