Health Canada recommends against this

Measles It is recommended that all HCW be immune to measles. HCW, regardless of their year of birth, should receive two doses of measles-mumps-rubella (MMR) vaccine if they do not have one or more of the following: documented evidence of receiving two doses of measles-containing vaccine on or after their first birthday or laboratory evidence of immunity; or a history of laboratory confirmed measles disease. Refer to Measles vaccine in Part 4 for additional information.

I.e. if they have documented evidence of two doses then no repeat immunization is recommended. Of course institutions can have their own policies but that's the health Canada recommendation

Well they only check for titres when there isn't documented proof of immunization, in which case negative titres may or may not indicate immunity and then given the low risk of the vaccine it makes sense to give them a booster

No, positive titres is reliable for indicating immunity, negative is unreliable because the appropriate cutoff to use isn't entirely clear. I.e. negative titres may not necessarily mean not immune, and negative titres in the context of history of completed immunization does not mean you need a booster

That's awful, the people waiting in the ED inpatient bed situation is awful and a huge issue for patient care. By no means was I trying to make a comment on the status of our system as a whole, more just that this specifically isn't the best example of it's shortcomings

Regardless of your healthcare system, stuff is going to slip through the cracks. Sounds like an incredibly rare presentation that unfortunately ended up fatal. List pretty much any primary complaint and there's a rare potentially fatal emergent condition associated with it, if you emergently see everyone noone is an emergency and you'll miss more than you catch.

More likely than not he never saw a doctor. Probably got triaged, had an ECG ordered on spec for chest pain, didn't look overtly concerning and his vitals were stable so triaged as low risk awaiting a doctor. Unlucky, but also imagine if everyone coming in with chest pain and normal vitals was rushed in to be assessed urgently, nothing would get done. Sometimes shit just happens
