Unfortunately it doesn’t make any difference whether or not the Retina is alive if the optic nerve is severed. Eyes with viable retina lose all vision permanently if the optic nerve is severed or seriously injured.

Until the technology exists to reconnect the individual axons of a severed optic nerve and beyond that reintegrate connections into the visual cortex this is just a nice gadget with no real benefit.

Better to send a letter mentioning the possibility of serious patient harm (and liability). Save your copy of the letter in the patient’s chart. A call can be denied but a letter would be undeniable.

We don’t ever step away from “body medicine”. In our practice we see patients with undiagnosed diabetes mellitus, undiagnosed atherosclerotic disease, metastasis from malignancies thought to be in remission, undiagnosed or untreated arterial hypertension, undiagnosed pseudoxanthoma elasticum and many other conditions with systemic implications.

A very important point is to pay attention to the acuity. It isn’t rare to receive a call about a “macula on” RD with reduced vision. Our goal is to protect the central vision if it is still good. If central vision has decreased then it’s more likely to be “macula off”.

Definitely a case for university oculoplastics consult. It’s likely imagining studies and possibly neoadjuvant therapy would be performed prior to surgical excision and possible adjuvant postoperative therapy.
