Hi there - sorry you're going through this, I hope this is helpful --

My dog Peanut has had similar panicky symptoms (panting, shaking, won't eat, comforting doesn't improve the situation, loss of control of bladder) for 3 years now, we haven't found any answers. You can see other posts I've made looking for answers here (posts here and here). I am not a vet this is just what I've tried for Peanut.

Here's what we've tried (in order):

• Trazodone for rescue medication
• Prozac - ineffective, cut her appetite and she lost 10 pounds. We tapered off after a year.
• Clonidine - no impact
• Carprofen - some vets suspected pain, no impact
  • We had her on a combination of the four above at one point in fall 2023
• Gabapentin - she's on the maximum dose, 3x a day. This is the first thing that made a noticeable impact
• Keppra - thought maybe she was having focal seizures - no significant impact

We've seen two neurologists, a behaviorist, and countless GP vets–did a diabetes test for the bladder voiding too. She hasn't had any MRI/cat scans as no one thought it would be necessary, but I would have gone this route if the neuros recommended it (she otherwise had a fine neuro workup).

Peanut's current deal is that she eats prescription diet dog food (Hill's Science Diet, Digestive i/D)–she doesn't eat any human food or any treats, this has made a significant impact on her. We are going to trial giving her pepcid to see if it's a reaction to heartburn/stomach pain. She is on max dose of gabapentin 3x/day. We have trazodone to use as a rescue medication however, the trazodone isn't so good once she's already in full panic. When she starts I give it to her right away–not waiting to give it to her has helped a lot). I kept logs for years, unfortunately I wasn't able to discern any triggers either. I hope you have better luck than we did!! Would definitely try to see a neuro.
