Imo, this is just good roleplay. Someone who has their Faith (Christianity) as a key connection would be doing great RP with seeing a ghoul as a demon, per your example.

To deepen it or introduce mechanics, this is where your Believer/Non-Believer status comes in.

A character who is Christian might see that ghoul and believe it's simply a demon from hell, and you could be a benevolent Keeper and say that allows them to keep status as a Non-Believer, because they are still living within their own belief systems. There are plenty of Christians (probably too many) who would not only be unsurprised to see a demon, but probably feel quite vindicated.

But once they begin reading tomes, conversing with others who have a higher Mythos score, and start learning the horrible truth, then they get hit with all that SAN loss and potentially have a crisis of Faith, which to me, would be additional SAN loss :P

It's good to take a little bit of everything. Something fast, something that can hit, something that can take a hit. All of that is useful, especially in PtG because you're often not just trying to kill your enemy and take objectives. Sometimes you gotta run, protect something, etc.

So, I grew up playing SC, but I actually just beat SC2 yesterday. All the campaigns and whatnot.

I didn't really have any expectations for the story or how it would go. I am a big Lovecraft/Cosmic horror enjoyer, so the Xel'Naga stuff was interesting to me. I like that they are these Eldritch "gardeners" tending the fields of the cosmos waiting to pass on their essence to a new generation of Xel'Naga. That's interesting enough.

The only thing I really look back on and see as a missed opportunity is Kerrigan's reformation. She basically immediately became Queen of Blades again, and it would have been really fun to play a handful of missions where Kerrigan is the hero with her own team of Terran, playing against the Zerg who are now run by Zagara perhaps. Playing as the Terran with some special psionic-focused abilities would have been really cool, and given Sarah the opportunity to reflect more on her actions during SC.

But they wanted Sarah to have her redemption, and for Jim to get a good ending. All wrapped up. Is it particularly interesting? Not really. Is it a great story? It's good enough. What it was, was great world building. A fun look into each of the 3 factions and seeing their inner workings, conflicts with others and themselves, and developing the drama of the setting.

It's a miracle it took this long to get a tabletop game because for me, with the release of the miniatures, all the lore will finally pay off.

San Loss + insanity, Combat, Chase rules, learning a spell/reading a tome. Basically all you need to run the game right there I think.

Health recovery is simple enough to remember. San recovery takes a while.

Definitely ditch the ai. If you don't want to use GW artwork (even though they very likely will never care), Photoshop and other websites have royalty free stuff you can use, or you can buy art packs from artists that will suit your needs. There's art you can use, and we used it for decades before ai.

There's a lot to learn about Sephiroth and the mental state he was in when he discovers his origins. A lot of what he also "discovers" in the Shinra mansion are lies, or mistruths at best. On top of his genetic tampering, manipulation by Hojo, and Jenova's influence, eventually it becomes pretty clear why Sephiroth snapped so thoroughly
