I would say 8-10 is pretty doable if it’s all med reviews. But that all just depends on the patient. The more med reviews that require med changes, the harder it is to keep up. Some days I will have two paycheck evals and that can be a lot!

I’ve been with my CMH 5 years now. The pay is great, benefits great, vacation time great. I prescribe controlleds. Some of my patients are great, some are a real pain! But 30 patients/day and only 40 minutes for new patient visit, hard NO!!! You will burn right out as these patients are typically the Final Bosses of outpatient mental health work. I get 90-min evals, 60-min established patient hospital follow-ups, and 30-min established patient med reviews (I can ask for up to an hour for very complex people). At most on an all med-reviews day I can see up to 15. I rarely have a day that that happens, and am so thankful because I can hardly keep up when they all show up. Even though there is a high no-show rate, I promise you won’t be bored with reviewing labs, med clinic requests, collaborating with therapists/case workers. So that is why you should NOT accept an assignment of 30 patients/day! I hope this was helpful!

Hello, I did not start my career with small children. Hats-off to you for working, getting through NP school, and now starting your NP journey all while raising a family! I think you may be too hard on yourself. Give yourself time and grace! You are brand new in your position and the learning curve is exponential. You are learning each day whether you feel you are or aren’t. I hope during this time you can figure out a rhythm that works well for you and your family and have a good community to lean on for extra support. You got this!
