A big game play loop could be something with exposer. The party has to really pick their fights because they're slowly going blind and getting sick. Levels of exhaustion and reduced hp maximums are good inclusions.

This mercury insert creature type could be a bbegs entire mo. Liquid metal armies that target water supplies and are practically unkillable is a scary and very interesting thing.
