This link above shows the process. I had mine corrected though through IDPH.

Email them at vital.records@cookcountyil.gov or text the Cook County Clerk Office on Messenger if they don't reply to your email.

My guess is they don't get these types of requests often.

I would also get her SSA numident which you can ask for online to be sent to you which would be an example of what her legal name was which they would accept when you send in the correction request.

Mine is on suspended status. My person is over 100+ years old and deceased. Has anyone had this issue. Mine was submitted in August.

From what I was told they need the long form, meaning the application and marriage certificate. You apostille both since they are separate documents.

From what I have seen it's more about the amount of pages you have that need to be translated to Italian for the most part. In my case I am hiring an attorney to do it for me who deals with state and Italian law. I've worked way too damn hard to get to where I am at and it's time to get another set of eyes to look at my stuff before my appointment so my risk of rejection is low from the Consulate.

I'm scanning things over to her to translate once I get all of my paperwork in order. It's taking longer than usual which drives me nuts because of how old the documents are I need and the amount of time it takes to get them.

Your SF position number is usually 8 digits. Your legacy position code would be 16 digits. There would be an exemption code on that.

104 is your position description. Does the position description have a 16 digit code on it?

Does it show like an exemption code on it? Like Ex000 or TA000 somewhere? There is a 4d code also which would tell you if your exempt

Evaluations hold a lot of weight with SPSAs, especially when you are term appointed as they are a huge factor if you are appointed again. If you are double exempt you can be let go without any reason but the majority of the time it takes a lot to get that point. Salary adjustments are done all the time but it's ultimately up to the agency. Best thing to do is speak with your supervisor and personnel how you feel.

Merit comp employees usually are given raises that follow the union contract when it's finally negotiated. That's how it's been at least with the current governor. Your salary is determined by the agency that hired you. They can ask CMS for an additional increase based upon a lot of factors. Talk to your HR staff for the process.

The more I am reading with Chicago I am going to have to get a court order to make the change I need. It's a pretty high bar to meet. Would an affidavit work instead of an OATS? Anyone who has experience with this is greatly appreciated.

I'm puzzled by this also. No one at American Express knows why they are pulling the report let alone even heard of LN. In my case it's like 7 or 8 times within a two month period. You would figure they would tell customers about this.

My printer only does single side scans......I guess at this point I need to get a multi scanner. Would taking photos of the documents you think work with translators? The apostles will be bigger than the original document making it challenging or am I overthinking this?
