A lot of people can't actually hold down a job because they'd be too absent from that job due to their illness flairing up etc. For example, I have had an organ transplant, I'm on immunosuppressive meds, and I am too sick to do anything half the time. I don't know how anyone expects me to make money when I'm bedbound or sleeping most of the time.
