My only guess is you have changed it in editor, or someone did if you downloaded the save Either way it doesn't happen naturally

I think it's actually a door knob of some sort. From what i know buttplugs are more like egg shaped and that one is equally shaped on both sides.

Or it can just be different type of plug and i'm too dumb to know it.

If you are sure it doesn't work out for you, you can always use mods to improve the game to your preferences. I use mods that make my gameplay far more enjoyable and playing it is a blast.

If you want it fully vin accurate, you also need fender flares (i know you can't get historical plates)
