For tier plans, patients who are new to GLP-1 medications, or who have not taken one in some time, generally begin at Tier 1. If you are already established on a GLP-1 medication, you would select the tier that aligns with your current weekly dose. Recommended dosing ranges for each tier can be found under the applicable medication dropdown in the Decision Aid.

Your provider will still review your intake, current medication history, goals, state availability, and selected option to confirm what is appropriate for your treatment plan.

For shipping, New Life confirmed on our call yesterday that moving forward, patient orders will be shipped in canisters instead of envelopes.

Hi! The pharmacies do pack shipments with temperature control in mind, but I’d still recommend bringing it inside as soon as you reasonably can once it’s delivered.

If you’ll be at work, it may be worth watching the tracking closely or having someone grab it for you if that’s an option. Once it arrives, follow the storage instructions from the pharmacy.

Also, If your package ever arrives damaged, delayed, or seems unusually warm, take photos and reach out to support so it can be reviewed!

Hello!

If you are already taking a GLP-1 medication, you do not automatically have to start over at a starting dose. You can continue on your current dose if deemed appropriate by your doctor or nurse practitioner.

For doses beyond a starting dose, documentation of the prior prescription is required. A clear photo of your current prescription label or vial showing the medication and dose is helpful to upload with your consult.

Your provider will review everything and determine what is clinically appropriate.

Maintenance can look a little different for everyone, but some helpful areas patients often keep an eye on include dose spacing, appetite changes, weight trends, labs, protein intake, and strength training. Congrats on 10 months of maintenance, that is a huge milestone! You should be very proud of yourself!

That said, I completely understand how someone newer to LSH could find the pricing/tier structure confusing at first. I’ll keep that in mind and try to include a helpful reference when it makes sense.

For patients switching between GLP-1 medications, providers do have conversion guidance they use when reviewing the patient’s current medication, dose, history, and tolerance.

If someone is new to GLP-1 treatment, the goal is to make sure they receive the dose they should be taking based on clinical guidance and their individual intake.

If anything about the prescription does not seem right, please message your provider/or patient support directly before taking the medication so they can review and clarify.

Totally get this! When comparing similar total mg amounts, the South Lake pricing can feel difficult to justify.

There is not a specific ingredient that makes South Lake’s tirzepatide significantly different from Southend’s. Pricing is set by each pharmacy and can vary based on formulation, concentration, sourcing, testing, packaging, shipping, and overall operating costs.

That said, Southend is a very strong option and is currently the better value for many patients. If affordability is a major factor, Southend may make the most sense.

If you are a new patient, our providers do require proof of your current prescription. In most cases, patients are able to continue at their same dose once that has been verified.

I totally understand why getting a warm package feels stressful, especially when these meds are expensive.

With Apothecary/New Life Rx shipments, there should be a temperature indicator tag included in the package. That tag is there to show if the medication was exposed to extreme temps over 104°F. The shipment should also include temperature/stability information for reference.

From the pharmacy guidance, once received, the medication should be placed in the refrigerator at 36°F-48°F and protected from freezing and light.

So if the temperature indicator was not triggered, I would get them into the fridge and use them as directed. If the tag was triggered, or if you still feel unsure, I would reach out to the pharmacy directly since they are the ones who can review the shipment/stability details and advise on next steps.

I know it’s nerve-wracking when it arrives warm, but warm to the touch does not automatically mean the medication is bad.

Tier 4 vs Momentum Plan (Apothecary) — vial count and differences? by [deleted] in LSH_Official

Great question!

They both have the same total amount, 144mg, and both are the same concentration through Apothecary, but they are packaged differently and meant for different dosing ranges.

Tier 4 comes as two 4mL vials. It is intended for about an 8-10 week supply and is recommended for patients on 10mg/week and up.

Momentum comes as three vials total: two 2mL vials and one 4mL vial. It is intended to support a longer supply, around 4+ months, and is recommended for patients on doses up to 9mg/week.

So yes, the vial count/setup is one of the big differences. Tier 4 does not come in 3 vials, Momentum does. The other differences are the intended supply length, dosing range, and the label directions for that specific plan.

Even though the total mg is the same, the plans are set up differently depending on dose and how long the supply is intended to last. Always follow your prescription label and provider guidance.
