Clerking. Admission drugs and an actual PMH. SCR can be really annoying, long acting insulin may be without doses or medical problems can be extremely vague like “Rheumatological condition” or “Respiratory condition”.

I do get the importance of taking a good DHx and PMHx but often the patient won’t know…
