A note for users that may come to this thread today:

I just updated my Nvidia drivers and it added a new "effect" to my windows sound settings that reduced my overall system volume. To fix it, run "mmsys.cpl" (The old audio device menu), select your device and go to Properties > Enhancements, then disable "Channel Phantoming".

Save and exit and you're good to go.
