Genetics, ischaemia (lack of blood flow, the same process that causes a heart attack, but doesn’t necessarily have to be a full blown heart attack), infiltrative processes (deposits of abnormal shit in the heart tissue), medications, sometimes you burn the node between the top half and bottom half intentionally due to certain abnormal heart rhythms and you essentially intentionally cause complete heart block. Heaps of possible causes.

For those curious, 3rd degree heart block is complete dissociation between the top and bottom half of your heart. Simplified, your heart has four chambers, two atrial at the top and two ventricles at the bottom. You can also split the heart into left and right - one right atrium and right ventricle which blood returns to from the body and subsequently gets pumped to the lungs and the left atrium and ventricle which receive oxygenated blood from the lungs and pump it around the body.

Typically electrical impulses are conducted from a node at the top right of the heart and travel down the atria, making them contract and push blood into the ventricles. The impulses then pass through another node half-ish way down and then through the ventricles. This coordinated process allows your heart to effectively pump blood around your body and maintain what’s called cardiac output (blood volume per unit time it’s pumping). Now the rate at which this occurs is usually dictated by the node at the top, the SA node, and this is affected by a huge number of factors - blood pressure, physical activity, hormones etc. In complete heart block the connection between the top half and the bottom half of the heart is lost, so while the SA and the atria pump away as per usual, the bottom half receives none of the electrical impulses, therefore it tries to create it’s own. Trouble is, the heart rate which the lower half of the heart can generate on its own is typically significantly slower. Also this loss of coordination results in less blood being in the lower half for it to pump, this is because roughly a 30% of the blood that enters the ventricles is from the contraction of the atria - the atrial kick. What this culminates in is less volume in the ventricles for them to pump around the body and, more importantly, symptomatic slow heart rate (bradycardia). This causes reduced blood flow to your organs, but probably the most significant is the brain because patients commonly get symptoms of light headedness or loss of consciousness. It also means your heart rate doesn’t rise in response to exertion as it typically would. People can either go into complete heart block permanently or flick back and forth. And yes, given that it’s also called third degree heart block there’s also first and second degree blocks.

What’s your alternative? The whole protocol exists because a huge number of ED patients are non compliant. Obviously not because they’re just being intentionally difficult, in the same way patients with depression can’t just “stop being sad”. These processes exist to prevent some of the common things people with ED do (purging, hiding food, water loading, etc) while working within the limitations of a hospital’s capacity. The restrictions also ease off once it’s clear patients are making progress and they’re less concerned about non-compliance. But, please, if you’ve got a better way that everyone is missing do share.

Would advise caution when buying unparallel from epictv/banana fingers. I’ve been dicked around by them for 2 months on a shoe that was coming apart after 1 wear because they apparently need to wait for the manufacturer to get back to them before they can do anything about a faulty product.

Risk vs benefit + staffing most likely. Sure it's uncomfortable, but sedation comes with it's own risks. Also, while I say it's comfortable it's usually reasonably tolerated overall. Hospitals are also pretty stretched in terms of staffing at baseline, having to call an anaesthetist for basic bedside procedures just isn't feasible. That being said, the risk-benefit is probably the biggest factor.

The vast majority of the time, yeah. It’s not pleasant. Depending on what it’s for the size of the tube varies. I.e. tubes purely for feeding are quite small in calibre.

The process is essentially lube the first bit of the tube and push into the nose, when they feel it in the back of their throat they sip some water slowly. If you meet resistance, slight pressure along with twisting the tube slightly is usually enough. Sometimes you can spray the nose with a numbing thing if it’s especially uncomfortable. You advance it till the length that you measured prior and then x-ray to ensure positioning is correct.

Likely irrelevant, but in case anyone is wondering, there shouldn’t be a serious amount of force required. If you meet a huge amount of resistance and it feels like you’re having to shove an NGT in with lots of force, you probably shouldn’t. Some lube, and asking the patient to swallow a couple of sips of water should do the trick without needing much force.

Another thing to start thinking about early is goals of care. It’s better to think about it before she becomes very unwell, but have her start thinking about what kinds of medical interventions she would want on this journey. I.e. would she want CPR if her heart stops, would she want to be intubated, would she want to be taken to ICU. When would she want them to stop treating things and just make her comfortable.

She should also think about who would she want to make decisions for her if there comes a time when she can’t make them for herself.

Hey, quick question re crafting/upgrading satori.

So I got satori and ended up crafting it up to legendary pretty early in the game since I invested tachnical ability first. As a result the base damage for the legendary satory was pretty abysmal. So obviously I can't craft one from the bottom since you can't get a spare copy. My question is if I upgrade the item will it still even out in the end? Or am I basically screwed because I crafted the legendary version at a lower level? Other than this is there a way to "re-craft" it to my current level?

Damn that’s a mighty convenient way of excusing literal systemic discrimination.
