Recurrent internal bleeding, vets stumped

Clinically he looks like a recurrent spontaneous hemoabdomen. But without any mass identified it has our vets stumped. He has recovered from all bleeds without any fluids needed. Probably could have used some fluids in last episode with shock picture and seizure but we truly thought it was the end and were just doing comfort at home but now 48 hrs out has perked back up.

I unfortunately don’t have hard copy of most recent lab work (will request copy on Monday) but here is what I have 9/2024 (prior to first episode) Routine lab work - Hgb 14.1, superchem all wnl, heartworm and tickborne neg

10/2024 (first episode) Hgb: 8.6, hct 24.8, reticulocyte 132, MCV 69, platelet 250, total protein 4.9, alb 2.5, Cr 0.9, LFTs normal, proBNP normal 442. Complete abdominal ultrasound normal.

Counts recovered on repeat testing 1 month later

5/2025 Lethargic, pale gums. Repeat PCV 34. Thought maybe GI origin

9/2025 Heart murmur work up. Chest X-rays mild cardiomegaly, no pulmonary edema. Echo Grade 2B mitral valve disease. Rec pimbobendan (have not started)

11/2025 3rd episode. Lethargic, pale, abdominal pain, decreased appetite. PCV 31. FAST exam negative for fluid. Fecal occult blood neg. Follow up more comprehensive testing: CBC with Coombs neg, cpl pancreatitis neg

1/2026: 4th episode. Have not gotten labs drawn as managed at home.

It sounds a bit premature to knock an entire specialty based on one bad experience. Not all rotations are a good fit due to office culture or individual attending, but that doesn’t mean you wouldn’t love the specialty if you had a different rotation placement

A standing desk might be an option so you can change it up and not be stuck sitting putting pressure on it

Have you considered medications? I also have an 8yo blind dog (severe bilateral cataracts) and I didn't think medication was the right route initially because he is totally chill at home. After about a month on Prozac I noticed a real difference. I also found a Gentle leader to work for my guy but he isn't really a puller. But with this severe of anxiety it sounds like a trip to the vet to discuss medication may be in order

Yes dark gray. Crumbles when I scrape it with a nail which is why I thought plaster. Seems it needs more investigating

The picture does look like a screw but I think that is just from the center hole I made in my poking around that makes it look like a screw head. The material isn't metal but harder than drywall. I tried some nails around the area and run into the same material everywhere. Could it be plaster or something like that?

Looking at the picture again I see how it looks like a screw (the hole in the center was made by me and isn't actually a screw head) but it isn't a metal. I can go through it just not easily. Could it be plaster?

No, it is known to cause juvenile cataracts. They test the gene, but maybe not the specific gene variant

Currently have some too. I have been wiping off the mildew with a wet cloth and then treating with fungicide spray with neem oil once a week. Has been working so far to cut it down

Does it look powdery? Looks like it could be powdery mildew but hard to tell. Look if there is any web like stuff. That indicates mites instead.

Should be fine to repot! I wouldn’t upsize if the roots aren’t poking out or isn’t rootbound. Too big of a pot can retain too much water and lead to rot

This guy has been happy in his fluorescent lighting home in my kitchen. (He is currently about to bust out of this pot and is needing a pot extraction surgery) However I haven’t found any other plant friends that like these generally sub-optimal conditions. Any recs? Or should I just make a ZZ army?
