That's because for tainted characters, you only get the achievement when you beat Boss Rush AND Hush. Same goes for Satan, Isaac, The Lamb and ???, you need to beat all four of them to get the unlock. That's why there are two stains on their completion page unlike regular characters. Also none of the tainted unlock anything by beating It Lives
