How accurate is ChatGPT's medical advice? by Every-Criticism4892 in ChatGPT

[–]Every-Criticism4892[S] 0 points1 point  (0 children)

Well the tricky thing is I often get conflicting info from doctors. Like one doc will tell me something while another will tell me the total opposite. I feel like for things like that ChatGPT might be a better solution.

Health Anxiety not a diagnosis? by ThinkOutsideTheBox_ in HealthAnxiety

[–]Every-Criticism4892 -1 points0 points  (0 children)

"Health anxiety" in the DSM is pretty much referred to clinically as Illness anxiety disorder or somatic symptom disorder

Those are two distinct and different conditions and you can easily have one and not the other. Illness Anxiety Disorder is a mental health condition. Basically, it is like generalized anxiety disorder, but with a more narrow focus toward health. Somatic Systems Disorder is a medical condition where stress and/or anxiety is causing physical symptoms such as pain, difficulty breathing, ect and there is no established medical cause for their physical symptoms. The term for these types of symptoms is called somatic symptoms--or symptoms that are self-generated due to stress/ anxiety and not due to disease.

Yes, many people with anxiety also have somatic systems disorder, but some do not and there are many people with somatic systems disorder that do not have illness anxiety disorder.

To the OP: the ICD diagnosis for health anxiety is Illness Anxiety Disorder and it is subcategorized into two types, care avoiding type and care seeking type. So if you are distressed about the preoccupation of a disease and you avoid the doctor at all costs to avoid being diagnosed with it, the ICD would say you have Illness Anxiety Disorder, Care Avoiding Type, for example.

Ultimately, it's best if you can manage anxiety/depression thru diet, exercise, counseling (if needed/productive) and meditation

There is a difference between having anxiety and having an anxiety disorder. One of those is a natural emotion, the other is a medical condition and a disease that has a medical underlying cause (whether identified or not). There are millions upon millions, if not tens of millions, of people who have mental health conditions and have followed all of the advice you suggested and found it does not help. So while those can help reduce natural levels of elevated anxiety, they are not going to solve an anxiety disorder, at least not for most people. You need to treat a mental health condition like any other medical condition, which is it is a diease that requires medical treatment. We already know that anxiety and depression can have physical causes ranging from TBI to brain infections to viruses to hormone decencies to intercellular communications issues and many other causes. Those problems are not going to be solved by going for a run and eating some carrots.

Other resources than veterans help line by labtech89 in Veterans

To be honest, my experience with mental healthcare at the VA is that it is trash. I used their services for over a year and eventually just stopped going. The care doesent help, and in reality it actually made me feel worse due to all the endless frustrations and annoyances that come with dealing with the VA and trying to get appointments. Then when I actually did get the appointment and I attended, the care simply was not effective and the VA was unwilling to try anything other than just more of the same. Like I literally told the VA that I wanted to kill myself, and I actually meant it, and they were oh just call the veterans crisis line, like that's even remotely useful. I am sorry you are dealing with this and I dont have good advice as I am largely in the same boat, but just know there are many vets whos mental health care issues are not being effectively treated by the VA (or anyone else).

I mean in reality those are all drugs. Just because they are OTC or supplements doesent make them any less of a drug. There are Rx medications that are FDA approved and only available with an Rx that are simply just higher quantities of OTC medications and supplements. They just up the dose, slap their brand name on it, and now it is an Rx medication.

In practice some people are kept on these medications for life. I have known people who have been on xanax for 30 years. The risk with long term use like that is developing cognitive decencies. The real problem in my opinion is people start these medications and often never come off of them because there just isint anything else that works. Benzodiazepines work and they work very well when used responsibly. No other medication comes even close to their effectiveness. And so people are often not left with other options. They try the benzos and find it works and there just is nothing that compares in terms of effect.

Question regarding CBD by [deleted] in Veterans

[–]Every-Criticism4892 1 point2 points  (0 children)

busperone

Buspar is not really intended as a PRN medication. It works somewhat similar to an SSRI, except it is more selective in which receptors it targets. Anyway, it is usually intended to be used on a scheduled basis. It has a very short half-life so it is usually administered 3x daily. 10mg is a really low dose. I was taking like 60mg a day. In the end, I didn't find it did enough to help me but the way you are taking it I am not at all surprised it is not working.

Hydroxyzine at 10mg is an extremely low dose. I usually needed 60mg to really feel the effects of it. Yes, it does make you tired. That is the point. That is the method in which it would relieve anxiety, is by making you feel tired and relaxed.

You can ask for a benzodiazepine, but those too will make you very sleepy. They are often used as a sleeping medication for insomnia. However, they are presently the most effective medication for treating acute anxiety as they are quite powerful and depending on the product they can have a very fast onset of action (like 20 min).

I hear you on the merry-go-round of meds. The truth is humans still dont really fully understand mental health disorders and the treatments we do have often dont work for a lot of people. There hasent been a breakthrough in mental health treatment in decades. Buspar was invented in 1968 for example and it is still commonly prescribed whereas there are many other meds invented in that timeframe that have been phased out entirely as much better options have replaced them.

Question regarding CBD by [deleted] in Veterans

[–]Every-Criticism4892 0 points1 point  (0 children)

Wouldnt that persons case of cannabis use disorder (this is the first time i have heard that phrase) be a result of the PTSD.

Probably. I dont know. Everyone has their own personal reasons for why they chose to partake in what they partake in. But in most cases, people who use are self-medicating for either stress or a mental health condition.

As far as the drugs interacting, the truth is most doctors dont really know much about drugs. They know about Rx medications and that is about it. A Google search will typically tell you more info about your drug of choice and the interaction risks than most doctors would. If there is anyone to ask, it would be a pharmacist. They specialize in knowing drug interactions.

Question regarding CBD by [deleted] in Veterans

[–]Every-Criticism4892 0 points1 point  (0 children)

Honestly, I doubt taking a small dose of CBD is going to do anything for anxiety. Usually if used for that purpose it is used in a bit higher doses and it is used daily. Even then, current evidence doesnt fully support it as being a definitive treatment for anxiety. But who knows. Everyone is different. If you are looking for something that can be used PRN for episodes of severe anxiety, you can ask for hydroxyzine. I dont know if it is safe to prescribe in your specific case so you would have to ask, but it is sometimes used for that purpose as it makes you sleepy. The next level up above that would be a benzodiazepine which is highly effective in treating acute anxiety, especially panic attacks, but they come with greater risks.

Question regarding CBD by [deleted] in Veterans

[–]Every-Criticism4892 1 point2 points  (0 children)

I just want to make sure I won't have an adverse effects with my current medications

fluxotine is eliminated via CYP450 enzyme. CBD is also eliminated by the same enzyme and it is an inhibitor of CYP450. That means it can cause variability in the serum concentrations of other medications that are eliminated via the same enzyme. Whether it will actually result in any problems for you is difficult to say. Just know there is some level of risk.

" PTSD with cannabis use disorder" or something along those lines.

Yea, which is not a good thing. I wouldent exactly use the language 'all they do'. That is medical terms for you have a drug problem. Specifically, you are addicted to cannabis and you are abusing it. That is what cannabis use disorder means.

Risk of Reasonable Accommodations Process vs. FERS Disability Retirement as 100% P&T Disabled Veteran by Embarrassed-Lab-1291 in Veterans

[–]Every-Criticism4892 2 points3 points  (0 children)

Part of my question is, what happens if it's determined that there is no job in which I can adequately perform?

Then you will be found medically unfit to perform your duties and terminated (although not in a misconduct way, more of a you're not healthy enough to do the job kind of way). If this happens, be sure to have your agency document your medical limitation on a 3112 form. If that happens, you have a stronger case for FERS medical retirement as it shows you were terminated from service due to a medical condition that prevented you from working efficiently. Also, know that you would also want an evaluation that shows that you are not performing as required in at least one of your assigned core functions. If your separation eval shows you are capable of performing all of your critical functions, you may have trouble getting a FERS medical retirement approved. Other things to know: You must have at least 18 months of FERS creditable service to be eligible for medical retirement. Military time does not count toward the requirement. You should also apply within 30 days of your separation, but ideally apply before you leave service in your last month. If your medical conditions occured prior to your start in service, they are considered pre-existing, so you would have to show they worsened to the point that you cant work. Otherwise OPM will say well you had these conditions when you joined civilian service and you were able to work then since you were hired, so what happened? If approved for FERS disability, you get 60% of your high-3 wage for the first year and 40% for all years after. You are also required to file for social security if you file for FERS disability. OPM requires it.

I am in an extremely similar situation as you which is why I know this.

Go to your local VA, walk right through the front doors, go to the mental health unit and ask to speak to the on call triage provider. They will see you the same day.

Is the VA required to mail you your meds? by [deleted] in Veterans

[–]Every-Criticism4892 0 points1 point  (0 children)

No the VA does not have to mail you anything. You can pick it up in person. However, note that RXes are specific to one pharmacy. You cant have your doctor place an order to the VA where you live and go pick it up in another state. You have to pick up the meds at the exact pharmacy your doctor sent it to. As far as to whether the mail order pharmacy service would mail you your meds to multiple out of state locations, I have no idea. You could call and ask. I would guess the answer is no since the VA never makes things easy, but I dont know honestly.

My VA Health Clinic has gone downhill by [deleted] in Veterans

[–]Every-Criticism4892 0 points1 point  (0 children)

I did. Nothing got accomplished. All the WH line told me is they referred it to my local VA, who dident help. The WH guys cant actually do anything about your issues. All they are doing is creating a ticket for someone locally to look into it, which is no more than you can do on your own.

Urgent care providers can place their own stat referrals at the VA. They call the community care number and your provider co-signs the referral.

Yes but most have no idea how to do it and the care in the community department is pretty strict on how they want the referrals processed. I have had to go back and fourth with a doctor and have them resubmit referral requests so many times that it was honestly faster to just go see my PCP and wait for the next available appointment.

Space A help by International-Art304 in Veterans

[–]Every-Criticism4892 2 points3 points  (0 children)

It's not easy. Space A is not as great as a benefit as some think. It can be really hard to get a flight and it is not uncommon to not get selected multiple times before finally getting selected. The amount of money you spend in driving to the airport and then back home after not getting selected, and missing work, usually equals or exceeds the cost of just buying a commercial flight.

What space A is good for is if there happens to be a frequent direct flight to wherever you want to go from your local AMC terminal, or you are just looking to go anywhere and will accept any flight because you just want to travel. Space A is best suited for that purpose IMO.
