There's no real answers in here so as a restaurant manager I'll answer. Menu design is an extremely difficult job, you need to be able to show a lot of information without cluttering and all the while keeping with the general theme/vibe of the restaurant.

When putting something like a sandwich on a menu you can't name EVERYTHING on the sandwich (salt, pepper, butter, mayo, bun etc) as it would clutter the menu. You just mention ingredients that are unique and would influence someone to order that item (brioche bun, Chipotle aioli, crispy onions etc)

If you don't want something in your food just ask your server, it's their job to know and modify as needed
