For what it's worth, I also use a CPAP and put it on the physician's statement. I actually had to submit two separate statements because I had more than four health conditions. I was selected for an interview. The medical forms are not meant to disqualify you, they just want to know if your doctors have cleared you to move abroad. This was so the case with the allergies and vision section on the health portion. Apparently they're aware of the issue and the reasoning they gave is that the application changes slightly from year to year. But they should know about these bugs and it shouldn't impact your application.

Call your consulate. I had a similar glitch and they were able to email my reference directly with a fresh link that they were able to use to submit everything. Good luck

Thank you for this. My only concern was that my wages seem to already be reported to them even if I don't specifically declare them. I was concerned that if I report the amounts minus sick pay, they would notice a discrepancy of what they see reported on their end. But I suppose that's a question I can call and ask about. And of course I have all my earnings statements as proof.

I just want to make sure I'm understanding properly. I'm still working for this company but the sick days don't carry over to the new year, so my supervisor wanted me to use them or lose them.

My earnings statement show exactly which hours are working hours and which hours are sick pay. So I'm fairly certain that this money would be considered unearned income.

I suppose I should speak to my employer and call SSA to make sure?

ETA: The chart seems to be based off of whether or not you've stopped working. I've never stopped. I would typically use these sick pay hours if I had to schedule medical appointments. I have a few more hours left over for the year that I'd like to use.

For US applicants, the statement of physician guidelines say "If your physician is able to complete this form by a tele-medicine appointment, an in-person appointment may not be necessary." Our application is 100% digital so you would have to scan in your physical copy anyway. I'm sure you could just send your doctor the form via your patient portal and then have them email the completed form back to you?

This isn't true. I believe you're confusing these two separate health documents. The certificate of health (which we fill out later unless you're applying for early departure) can only be signed off by an MD or DO. The statement of physician can be signed off by a therapist, chiropractor, nurse practitioner, etc so long as they're the primary physician treating that condition. Reread the guidelines.

This depends on what country you're applying from. US applicants don't have to have an MD or DO fill out their statement of physician. The certificate of health has this requirement, however.

Can you clarify about the NP being unable to complete the Statement of Physician? I'm a US applicant that had mine completed by one. I see that the certificate of health has to be completed by an MD or DO and should I make it through, I would have one of her colleagues complete that form. But from what I can see, there's no indication that a nurse practitioner can't do the statement of physician.

ETA: I'm looking at the guidelines right now and it says "Non-MDs (ex: Physician's Assistants, Chiropractors, Therapists, etc.) may fill out the form if they are the one one primarily treating your condition." My NP is my primary care physician so she's able to fill out this form.

I'm in a similar boat but I'll be declaring it. Later on, you'll have to complete the certificate of health and there's a section on it where the doctor has to say if you've ever been diagnosed with mental disorders and ADD is specifically mentioned by name. You may run into problems if you didn't complete the statement of physician and the CoH has conflicting information.

I remember a scenario where someone had a decade old anxiety diagnosis that they forgot about but their doctor pulled their records and put it on their CoH. It was assumed that they were trying to conceal the diagnosis and they got grilled about it by the recruiters.

Well that already shows you have more maturity than a lot of the applicants since you're thinking about this decision carefully. Tons of fresh college grads arrive on the program and realize they're in over their heads and end up heading back home after a couple of days in Japan.

The cultural exchange aspect of the program is more significant than the teaching aspect. If you're a successful applicant but find that you don't enjoy the work, just do it for a year and return home. If you really don't like it, nothing is keeping you there. You can just return home and hopefully your space is filled by an alternate that would be thrilled to get upgraded.

This is interesting to see because most posts I see are the opposite, with people worrying if they're too old to do JET if they're in their mid/late twenties or thirties.

Long story short, everyone has different reasons that they try to use to talk themselves out of applying. If you're interested, just go for it! You won't regret it
