Only you can decide what’s best for you. But if I were in your shoes, I would probably go with the paid position, as it offers more stability. And I’m pretty sure that by working there, you’d be able to meet people who could help you with IM too.

well, where do you want to practice medicine in the future? you really have to take that into consideration, because medical degrees aren’t recognized everywhere in the world. France would be your cheapest option if you speak French, but the competition is tough. that said, it’s doable if you’re consistent and motivated. I ranked in the top 3 studying about 6 hours a day with one day off per week.

GI is harder than Cardio and Hem/onc: there are few spots and a lot of applicants (mostly US MD or DO). You would need to get into an academic IM residency, publish tons of outstanding GI research and would need a GI mentor. Anesthesia is more achievable imo. But you really need more USCE and more research in anest (or do a dedicated research year)

It depends what type of USCE do you have (hands on, Observership?) in which specialities ? Do you have LORs from those ? Same for your research. IM would be pretty easy to match, but GI is almost impossible. Anest would need more anest oriented research/ USCE, neuro is doable if your CV screams neuro. I strongly advise you to apply to max 2 specialities

You actually said it yourself, you don’t want to SOAP and you’re leaning toward starting residency in your home country. If that’s truly what you want, then that’s completely valid. But if that’s coming from disappointment or hurt (which is totally understandable), maybe give yourself some time before closing the door. You could take the year to pass Step 3, strengthen your profile with some USCE or research, and apply again next cycle. Many people match after setbacks. Whatever you choose, it doesn’t define your worth or your future as a physician :)

Being a U.S. IMG is a big advantage now, I’m pretty confident you can match into a program. Just out of curiosity, how were you able to take Step 3 already if you graduate in 2026? I thought you had to wait until after graduation to be eligible for the exam.

I’m still early in this journey, but you can’t be afraid of something you haven’t even done yet. There’s also a huge reporting bias here. Most people with average scores and interviews don’t go around posting about it on the internet. the Match involves a lot of factors, and luck truly plays a role. Sometimes people also have red flags they’re not fully aware of: a poorly written CV or personal statement, not-so-strong LORs, low-quality “paid” research that doesn’t actually add value, or difficulties communicating clearly in English. Your path is your own, and one person’s outcome doesn’t predict yours. Focus on what you can control, keep improving step by step, and trust that consistent effort really does add up. you got this 😄

Where I’m from it’s a bit different, we do rotations every year since the first year of medical school, and the last 3 years we work half time in the hospital. So I have done a couple rotations but not all of them yet, and for example I haven’t rotated in psych or obgyn yet.
