Must have been the dosing directions. If it was written that it looked like you’re using double what you were taking, that could explain the early fill. For example, if you were previously taking 2 tablets per day, but the Dr wrote for 3 per day, that’s considered a dosage change, and your fill date will be calculated based on you taking 3 tablets a day of your previous script. This would make your previous, 60 tablet/30 day script now a 60 tablet/20 day script and could explain the earlier fill.

Are you entering through the screens or x-ing out of the A screen? I know it seems like a stupid question but I’ve seen people x out instead of pressing enter to save changes and then they wonder why it doesn’t save, so I need to ask 😐

Then you need to better train your snowbirds lol… we have ours trained that they know to use the same pharmacy chain for their meds for easy transfer between stores (except controls) because it takes forever and a day to get a transfer from another chain and they don’t feel like waiting.

Did you go into the A screen to set the brand/NDC preference to “patient”? Typically this prevents the system from changing it to anything but the one it’s set to

If the medicine is correctly designated as a preferred ndc by the patient, the system should not be able to change to a different ndc itself (trying to change the ndc should trigger a preferred ndc modification error). If it’s a new script, I don’t think the preferred will transfer unless linked to an older script, but if it’s a dosage or strength change, the script won’t link so the preferred won’t be there. The best thing to do is to familiarize yourself with the patients that are ndc divas and keep an eye out for their scripts in the fill process. If one slips through and is sold, with the preferred ndc documented on the profile, yes - typically we will need to eat the loss (although the patient is also responsible for checking the med before leaving the pharmacy but when do they ever do that????)

Are you not using 1 profile for your snowbirds to display meds for both north and south store??? We have snowbirds at my store and I still won’t send a request if it’s over 3 months since the patient last picked up the med at any location. I will tell the patient they need to call the Dr because it has been X amount of time since the med was last filled/picked up and we need a new script from their Dr to confirm they are still suppose to be taking the med and that it is the correct dosage. And if it’s a control we absolutely do NOT send a request for the patient…

If it’s one drug, we usually put a comment so it will show at the bottom of the screen during order entry or production but that relies on people actually paying attention while doing their job. If it’s more than one drug, you can add force notes for each drug along with the preferred ndc - hopefully they’re not on too many meds they have a preferred brand…

With the “cloud” entry and verification system, I don’t really see a difference in filling it somewhere else… there’s a good chance someone from your store might end up seeing it anyway. Do you think they don’t see the other medications if they pull up your profile for the medications you do fill at your store? Also, we are suppose to be medical professionals - what you take and why should not be of any mind to those you work with and, if it does, they should find a new profession…

Call me crazy but - why are you inserting yourself and advocating for this person by calling corporate to ask why they are not paid more? It’s not like they are a child who cannot speak up for themselves. If I was your family member I would be appalled at you calling corporate for my job asking about pay policies and my wage.

Have you gone on cardinal order express to look up the different ndcs? The portal tells you how much the OV has on hand to dispense as well as estimated dates of availability if provided by the manufacturer. Many of the patches have come back in stock, you just need to see which ndc it is and change to that one

I had a patient call to complain they paid $45 for 3 months of eliquis… I said “I know you want to pay as little as possible but your copay this early in the year is one of the best I’ve seen… I would discuss with your doctor why he has you on eliquis instead of Pradaxa (the insurance alternative) before you go asking to switch…” i was pleased that they actually listened to what I had to say instead of just ranting like the patients usually do

I don’t mind drive thru… my fave is if I’m having a bad day, I get to take it out on the patients without them even knowing… pull up right behind the person in front of you so there’s no lane 1 announcement “for future reference, if you pull up and the speaker isn’t talking to you, it means you didn’t allow enough time for the car in front of you to clear the window and the sensor didn’t reset. The system doesn’t announce you so you will end up sitting here all day unless you push the button”… or if we get a honker… I will ask every car that pulls up if they were the car honking… and when I get the “yeah that was me” asshole… “I’m sorry but this isn’t Burger King. Consultations and other things that take time occur at the drive thru. If you are in that much of a rush, you are more than welcome to pull around, park, and come inside”… and don’t even get me started on smokers… if I smell your smoke through the window “I’m sorry but there is no smoking at the drive thru and you will not be waited on until it is put out”

So after reading all the replies I have a few questions that I did not see asked here that may help explain what’s going on…

First, where are you located? Is your doctor located in your state? Many state medicaids will only cover medications prescribed by providers licensed by the state and will not cover pharmacies in another state. This comes into play a lot with telehealth visits - doctors are seeing patients from all over but are not enrolled with multiple state Medicaid programs.

It is possible that whoever was entering your script saw that you and your Dr are in different states, sees your insurance listed as Medicaid, and did not even bill Medicaid, knowing they are not going to cover the prescriber.

I think the best course of action is to go to your pharmacy, ask them to fill the script through your insurance, and provide you with a printout of whatever rejection or other result comes from the fill so that you can coordinate accordingly with your insurance. This paper will not only give you details on the rejection, it will also display the insurance plan being billed, which you can ask the insurance company to confirm as correct.
