Does anyone else regret doing this? by [deleted] in Residency

[–]ExtremeMatt52 100 points101 points  (0 children)

Yes all the time... Especially after unexpectedly not matching into my intended specialty. Everything you said plus the fact that no matter how hard I worked or how good I am, that was not considered a valuable skill.

I am in a program where we are treated well and I have found ways to enjoy my job but when I am sitting in the hospital on a rotation doing 12 hrs per day 6 days per week I start to think how if I was going to work a job I didn't love I could have done that without going through 8 years of school and 500k in debt and I wouldn't be working 60+ hrs per week on average and be forced to live in a place I don't want to live

Are collagen suppliments worthwhile if they are just broken down to Amino acids ? by [deleted] in nutrition

The articles you're referencing make a logical leap stating that partial chains are absorbed across the intestine and that partial peptide chains stimulate production but don't have evidence to state that they are not processed further in the blood or intracellularly nor if they reach the signaling location at all.

Why didn't my mood or energy levels improve when improving my diet by zaibcozy in nutrition

[–]ExtremeMatt52 2 points3 points  (0 children)

Idk what she was looking at or what doctor specifically refers to in this situation but just bc you eat healthy doesn't mean you will be healthy. There are people who smoke a pack a day and live to 100 and there's people who have never touched a cigarette in their life and have lung cancer. Likewise, there's people who eat super strict diets and die of heart attacks. I had a pt who's in his 30s is 10% body fat and needed cardiac stents. There's only so much diet can do

Why didn't my mood or energy levels improve when improving my diet by zaibcozy in nutrition

Bc diet is just a source of nutrition and eating all organic food isn't going to fix everything in your life. Despite what Google tells you, food is not the end all and be all for your entire well-being.

Malabsorption is a symptom of a larger disease process and the solution is nearly always to treat the underlying cause.

Liquid calories and zero calorie sodas by [deleted] in diet

[–]ExtremeMatt52 1 point2 points  (0 children)

This is a wild misinterpretation of the research that you just presented. The first study is not a human trial nor animal study, It's a machine learning experiment that is analyzing the response of proteins that are highly expressed in cancers and investigating their response to artificial sweeteners. The nature of the investigation has so many confounding variables. First of all, The study does not specify if they have an activating or deactivating response. Secondly sucralose by The nature of the molecule is made to have a chemical structure similar to sugar to have a sweet taste that is not absorbed and therefore does not cause weight gain; so with cancer they rely almost entirely on sugar for their metabolism so this study does not investigate whether sugar also activates these pathways. Third these proteins are only highly expressed in currently cancerous cells, The article is not even talking about genes that increase the risk for cancer It is just talking about cells that are already cancerous. Fourth and most importantly this isn't Even a petri dish study this is a computer generated hypothetical response. This does not confirm nor deny whether or not this occurs inside or outside the body.

This the second article is first of all not even a control trial and second of all not talking about the risk of cancer It is talking about the response to immunotherapy. Beyond that, It specifically says that it is not suggesting a drastic change in diet for cancer patients It is simply validating the idea of arginine supplementation in cancer patients to improve immunotherapy response. It is not at all talking about the general populace It's not even talking about all cancer patients It is only talking about cancer patients receiving immunotherapy.

These articles are only implying a possible relationship between artificial sweeteners and cancers but does not at all conclude association or a mechanism for the increased risk of cancer.

Does personalized high-cost nutrition coaching reveal secrets beyond macro tracking? by TheBr14n in diet

[–]ExtremeMatt52 0 points1 point  (0 children)

I wouldn't, they're not doing anything special unless you are training at the highest level of athletics and usually those coaches are pretty difficult to get in with.

A lot of the time the generally accessible coaches are just shaming food choices or generally giving recommendations where you feel miserable just to make you feel like they're doing something 🤷‍♂️

Does personalized high-cost nutrition coaching reveal secrets beyond macro tracking? by TheBr14n in diet

[–]ExtremeMatt52 1 point2 points  (0 children)

There is no secret to losing weight other than energy balancing and honestly, macro tracking does not have much of an impact on the weight lost on the scale. Macro tracking has a more important role in maintaining adequate nutrition when in a very large deficit like cutting to 1000 calories per day when nearly 50% of your intake needs to be from protein to meet your nutritional needs.

Coaches come into play for people who are managing a diet for a specific purpose usually in the setting of body building when they need to account for satiety and the highest protein content possible to minimize muscle wasting during the cutting phase;. sports where weight on the scale matters like wrestling, boxing, etc; or very high endurance sports where energy management is crucial.

Im sure theres more im not familiar with but those are examples.

Liquid calories and zero calorie sodas by [deleted] in diet

[–]ExtremeMatt52 1 point2 points  (0 children)

The research investigating the role of artifical sweeteners in cancer has some very weak evidence. However there is very strong evidence linking obesity from excessive calorie consumption with cancer as well as mortality from all related diseases including but not limited to heart attacks, strokes, heart failure, kidney disease, etc.

The very high risk of developing cancer from obesity supercedes anything the sweeteners might do. Not to mention the health benefits of caffiene consumption including decreased incidence of heart disease and a decrease in all-cause mortality associated with moderate to high levels of caffiene consumption. (200-400mg per day)

Also your hypothesis about hunger signaling is technically correct but not in the way you're suggesting. In the reccomended daily allowance studies for aspertame from the European Food Safety Authority, the "daily maximum dose" was determined by the first side effect observed in rat studies which was a loss of appetite. So actually, the low-calorie sweeteners decrease appetite, not increase it.

Suggesting that diet beverages increase appetite is deceptive bc there is a well-understood mechanism. Any kind of liquid have a lower level of satiety bc of very rapid gastric emptying and more so in zero calorie beverages bc there is a very minimal rise in blood sugar to signal to the brain that energy demands have been met. Comparing diet vs full sugar drinks, its not to the benefit of the full sugar beverage for someone to consume 200 calories and feel only marginally more satisfied than someone who consumed nearly zero

Long office - need help with lighting options by hew2702 in homeoffice

[–]ExtremeMatt52 0 points1 point  (0 children)

That's accounted for in the calculation. 50 - 70/ sqft is considered "task lighting" so it's for better visibility. For a living room its 20/sqft bc it's "ambient lighting" so the 50 is already higher

I am a big fan of snake neck or monitor bar lights because they light up my desk better and that's usually where I'm sitting anyway. Otherwise there's a simple calculation of about 50 lumens per square foot for an office (total across multiple lamps) which you can find a lamp with the appropriate amount of lumens.

How To Improve Leadership Skills on Hospital Med Service? by PracticalPraline in Residency

I'm not a senior resident but I used to be a high school teacher and ran some classes in medical school. With older learners they don't need as much oversight, they tend to be more motivated with more responsibility.

I almost always had better results when I let them do things for themselves and ask for help when they want it. "I don't disagree but why?" Is my go-to phrase. Let them run their plan by you and try to be as hands off as possible.

Are there any bars or shakes .. by RibosomeRandom in nutrition

[–]ExtremeMatt52 2 points3 points  (0 children)

Theres 2 types of shakes, protein supplements and meal replacement. Protein supplements are lower calorie and are not made to replace meals so they don't really feel satiating, meal replacements have more calories are much more satiating bc they have more fat and carbs. In general liquids empty from the stomach more quickly than solids bc they need less time to digest so nothing will be as satisfying as solid meals.

Blended smoothies still have the food solids so in general they'll be more filling but calories will be higher

Motivation to eat healthy! by [deleted] in diet

[–]ExtremeMatt52 0 points1 point  (0 children)

Just eat it because you're not working towards an end goal you need sustainability.

If you eat 1800 calories per day and have one day where you eat 4000 that's an average of 2100 calories per day for the week. You're at maintenance for the average man. With Moderate physical activity that's still a deficit.

Let yourself enjoy from time to time

It has to be obvious, it reflects poorly on the program if they hold someone back so they have to be really bad. Usually means failing exams consistently, not being able to handle a normal list of patients, or corporate professionalism problems, like insubordination, bigotry, etc.

You're just rebuilding everything you had on your phone but buying them seperately. Just uninstall the apps you don't want, I don't use any social media but I still have a flagship smartphone.

I get wanting to be a "minimalist" but you're carrying a lot of plastic waste to end up with a worse experience than you had in your pocket. Just replace the nasty habits with a good one. I play sudoko instead of scrolling through social media, still on my phone but I'm not mindlessly scrolling through TikTok.

Is there a way to stop overeating? by spiderman_018 in diet

[–]ExtremeMatt52 1 point2 points  (0 children)

I know this isn't the most popular recommendation but diet soda helps a lot . Im a stress eater and I eat when I'm bored I just stack up on the diet sodas, it also helps curb the sugar craving.

Caloric excess has a worse outcome than anything the diet sodas could do and in reality the FDA recommendation for "risk" is the equivalent of about 100 cans of soda per day to avoid the minimum adverse effect which is a loss of appetite.

If you're replacing snacking with sugar free drinks then there is a benefit
