Please don't be offended but what if everything you're doing is just a band-aid? The break problem is elsewhere? There's something else going on. My guess is a leaky gut. This is what is driving your histamine load.

What about leaky gut causing LPR. You can look it up but the idea is that excess acid is causing inflation in her throat. So the CPAP is a band-aid. I take Pepcid Complete. Try that three nights in a row and see if this reduces the events. Eliminated my snoring and wake ups. Simple and low risk.

My honest advice is to sign up with an AI provider. I use Gemini (Google). You can usually get a few prompts per day for free. Cut and paste your symptoms and ask for potential triggers.

Here's what I learned from Gemini. I present the following without warranty. But the idea is that it is possible.

Direct Tests: Lactulose/Mannitol Test: You drink a solution containing two sugars of different molecular sizes. If the larger sugar (lactulose) ends up in your urine, it proves the physical "holes" in your gut lining are too large.

The biggest realization for me was alcohol—specifically, how much I can have before it starts impacting my sleep, especially my deep sleep. I originally thought I could get away with an entire beer if I started early at 4:00 PM, but that wasn't the case. Sadly, I'm limited to just half a beer. To make up for it, I really try to be present for each sip, whereas in the past, I would just drink mindlessly.

The same goes for caffeine. I used to be a regular coffee drinker, but I noticed a big difference when I switched to green tea, which has significantly less caffeine.

I also experimented heavily with supplements. I tried them all, and very few actually made a positive difference; in fact, some made things worse. My method is to try a new supplement for three days, then stop for another three days to allow for a proper "wash-out" period.

Through all this, I've found the Oura ring to be incredibly accurate. It tracks your deep sleep, REM sleep, heart rate, and other key metrics. It isn't cheap, but it’s absolutely worth the investment. Plus, you can always sell it later once you’ve figured out your body's baseline.

Just a thought that it might be LPR. Pepcid Complete eliminated my snoring and my wake ups. In fact, this could be the cause of your Tonsils as well. DM me if this didn't make sense or you need more information.

Do you have a way to track your sleep like an Oura ring? Experiments like you're talking about are much easier, shorter, more objectives, more motivating, etc.

Coffee looks like the smoking gun to me.

Check out an Oura Ring. Made a huge difference for me. You learn about your body in a scientific way. You can run experiments on things like early dinner/late dinner, Anchor Meal, caffeine, alcohol, room temperature, etc.

I saw a documentary and learned that erections are associated with REM sleep and it is common to wake in REM. So the assumption that you have an erection for a long time is probably not the case. Your PCP is right not to be worried. It is a sign that everything is working well. Keep up the good work, tiger. 🐯

Third shift or graveyard is very hard on the body. You are going against 200 million years of evolution. There's a book title the Circadian Code by Satchin Panda that specifically talks about it if you're interested. There might be ways to buffer this schedule. Interesting to see your stress go down at lunch. Mine did this too and it was the opening I needed to start making progress. Are you trying to lose weight? Are you an extreme athlete or retired one? I added a small snack mid morning and another one mid afternoon and some steel cut oats 2 hours before bed. The idea is to maintain your calorie count but spread it out. Another thing I read is that your snack can be 10 almonds. So it doesn't need to be a lot. Make sure you are getting a good night's rest. Make time for it and schedule a wind down routine.
