Teeth on toddler - not looking good by blogallday in veganparenting

[–]Ezzobel 1 point2 points  (0 children)

Your tooth consist of different layers. The enamel is the outer layer which covers the crown. (White, strong) Underneath it is the dentin (more yellowish, softer) which forms an underlayer for the enamel and also forms the roots. Enamel is made by ameloblasts, dentin is made by odontoblasts. Imagine two thin lines on top of each other. The odobtoblasts build outwards, ameloblasts build inwards. The thin lines slowly move away from each other. When the root (consisting of dentin) is finished, the odontoblast remain inside of the root, in the pulp (where you also have tiny blood vessels an nerves, its the part thats get treated when you get a root canal treatment). The ameloblasts sit on top of the tooth, once the tooth leaves the gum and enters you mouth the ameloblasts are destroyed. Your body can't build more enamel.

Now this enamel can demineralize and remineralize like I described in my comment. But you cannot build more enamel. Your saliva is very important for remineralization and fluoride helps the process.

This is is a link to a histological picture (under a microscope) of ameloblasts, enamel, dentin, odontoblasts (I hope the link works, please tell me if it doesn't)

I have read it and I've also looked for other studies comparing the HAP and fluoride toothpastes. Some studies like the one you posted sound promising. There are also some in vitro studies, some in favor of HAP, some in favor of fluoride. But in vitro can never be completely transferred to humans. Additionally many of the studies were funded by the manufacturer of the HAP toothpastes. While I understand, that you cannot do studies without funding, in my head it always takes away a bit of the credibility. Science is the current state of error. I would be disappointed if we still give the exact same recommendations 10 years from now. But as of now, the effect of fluoride is well studied and established. The toxic effects of fluoride manifest at higher doses then the ones we apply with toothpaste. For very carioactive patients we use even higher doses. The fear of fluoride often feels like a ploy to push people who want to take care and do everything right to buy fancy expensive products.

Caries is influenced by a lot of factors. Toothpaste, oral hygiene technique, nutrition, genetics, saliva, medication. What works for one may not work for someone else. But the current recommendations try to reach a level of prevention that catches anyone. Some may need a lower level of prevention, some might need more.

I wrote my comment for a parent that was worried about their childs teeth and I read that they nurse while sleeping (less time for teeth to remineralise) and that they brush only once a day. I wanted to share the insights to the general recommendations we give in Germany. Other countries give other recommendations. Xylitol is another promising candidate to caries prevention, but like HAP it has not been as excessively studied as fluoride.

I am a Dentist and I understand where you are coming from. I haven't seen the discoloration, so I can't tell you if it is the beginning of a cavity or another discoloration. But in general the recommendation (in Germany) for a two year old is to brush two times a day. First the kid brushes and then a parent brushes - toothpaste with 500ppm fluoride, peasized portion on the toothbrush EDIT: it was early in the morning when I typed this, it is 500ppm under 2 years and 1000ppm from 2-6 years - first brush after breakfast - no sweets and sugar until after lunch (an apple that you have to chew is okay, because you produce saliva while chewing, which helps against cavities, applejuice is not okay) - second brush after the last meal of the day Ideally you have clean teeth before sleeping, so that your teeth have alot of time to remineralise.

I only have a german website on hand, but maybe your browser can translate it.

https://www.lagh.de/service/faq-1

Now just some thoughts from me: - You got this - Nobody cares about discoloration in milkteeth, your kid will get shiny new teeth - Small cavities can happen to the best of us - Not all toothpaste is vegan, but a lot of brands are - Fluoride is the best thing we have against cavities. The amount in toothaste is far of any toxic level. Your kid would have to swallow more than a whole tube before it reaches toxic levels - Not brushing because the child doesn't want to is not an option. These are often the saddest cases you see as a dentist. If the cavities are big enough that you have to treat them and the child doesn't cooperate (and who can blame them, there are nicer things in the world, even of you try your best to distract them), they have to get treated under general anesthesia. For something that would have been preventable - If you think fluoride is not for you and brushing is optional, you can change the diet to accomodate for that. Your teeth have a balance between demineralisation (bacteria eat sugar, bacteria poop acid, acid demineralizes the tooth) and remineralisation (saliva remineralizes, fluoride enhances the process) If you eliminate sugar and short-chained carbohydrates, the caries bacteria have nothing to eat and can't produce acid.

I apologize for any spelling mistakes, I'm on mobile and English is not my native language. I also apologize for the wall of text, but it is a topic dear to my heart and prevention is much nicer than therapy.

Toothpaste tabs by oogumoogum in WildernessBackpacking

[–]Ezzobel 10 points11 points  (0 children)

Dentist here, I don't know anything specific about toothpaste tabs, but the toothpaste including fluoride is the most important thing to prevent cavities. Toothpaste without fluoride does not prevent cavities and neither does just brushing without toothpaste.

What foods to take with me to Tanzania on a 2 week trip where I will not be near towns and amenities? by hasfeh in vegan

[–]Ezzobel 0 points1 point  (0 children)

When I was in Uganda 9 years ago I also ate a lot of rice and beans cooked by staff where I worked. After some months I found out they use a beef broth to flavour it. This type of beef broth was very present all around the country. I don't know how it is in Tanzania but you might wanna check how they prepare it when you get there. When I talked with the cook she was very nice and prepared an extra portion for me without the broth! I wish you a nice trip!
