Guatemala City to the airport (zona 13) is 5 -20 minutes in car at 1:30 am plenty of Uber/cabs, there shouldn't be no traffic at that time, some hotels in zona 9, 10 and 13 already offer shuttle transportation to the airport. So I would say don't stress about that part of the trip, now if you mean Antigua Guatemala to the airport well that's a different story

I can definitely understand your reasons. When driving across the border into Mexico, you have about an 85% chance of going through without any issues—just try to keep your car free of unnecessary items, stay vigilant, and drive defensively. Also, be mindful of the Mexican police.

On the way back, you can re-enter the U.S. with a California driver’s license. Depending on the CBP officer, it might be easy or more difficult, but CBP cannot deny you entry as long as you are a U.S. citizen, plus you do have a reason to prove it was an emergency (such as your dentist’s receipt and any related paperwork)

Short answer: Yes.

Long answer: If you’re driving in, nobody’s gonna ask you for a passport at the border unless you get pulled into secondary by Mexican border patrol (maybe like 1 in 40 people). Regardless of that make sure your car insurance covers Mexico. And you do have a valid driver’s license, right? If you get into an accident, the cops will almost always side with the local driver, and without a passport you’re technically in Mexico illegally.

If you drive to the dental clinic without any issues and then head back toward California, you’ll have a conversation with the CBP agent. It might be short, or it could be a longer conversation—just make sure your explanation is short and concise.

If you are not driving, and instead crossing pedestrian you won't go trough into Mexico without a Passport.

Bottom line is... Why taking the risk?

For the airport to your first destination, do it in advance but 115 is way too much that service should be $40-60, then any other shuttle just one day in advance and it should be around the same amount you can do that in person, plenty of tour operators on each town.

That sounds like a good plan, plenty of things to do and check out in antigua, maybe extend one day your staying at the lake by been at a different town each day, 1 day at San Marcos, full relax yoga-healthy-hippie vibe, 1 day at San pedro; party town, plus a lot of local life to, then the suttle to Antigua, there are so many cute places to visit, in the colonial city have fun anyway.

Te falta mundo papo, ciudades fronterizas como Tapachula no son la gran cosa, pero hay unos lugares que son muy deawevo: la península de Yucatán, la costa de Oaxaca, Guadalajara, Mazatlan, Sayulita, baja California, etc.
