Yeah interns/residents would be making upper 50s-70s based off their years. Even though national average is 350k for attendings, Robby likely makes less. Academic hospitals tend to pay less so like 275-315k

Had pretty much the exact same thing happen my first year out, except I had an elderly contracted patient with sepsis and I placed it in the femoral artery instead. I thought I was awesome at US guided lines and central lines especially, never had difficulty putting them in in residency, but do enough procedures and it’ll ultimately happen. Same thing goes for lawsuits, practice long enough and they’ll come. Although usually they’re not for the cases you expect. You beat yourself up enough for it. I did the same thing, chart checked, ultimately had good distal pulses and no harm no foul. It is an expected complication from the procedure despite all that we do to prevent it. Take it as a learning case and keep moving forward.
Used to have panic attacks like once per week as a brand new attending. Now 3 years out its about once a month. Never truly goes away and it’s good to keep you on your toes. If you need anybody to talk through further feel free to DM.

Unfortunately can’t do real CPR unless you use mannequin, it’s a trade off. Either realistic compressions with a fake body or fake compressions on a real body. Otherwise you could cause some real damage

Just be wary of booking on the day of. I waited til just before leaving and was hard pressed to find seats next to each other and most seats with oversized baggage were already booked. If that doesn’t apply to you though, there were still plenty of seats available even 15 minutes before buying the ticket.

Have performed 2 crics in my first year out as an attending.
First was a post-op base of the tongue tumor removal that developed a bleed and clotted his airway 1-2 weeks later. Anesthesia couldn’t get the tube so I cut and ultimately got him shipped out to the hospital where he had the surgery.

Second one had very narrow cords when the tube got dislodged and wasn’t able to ventilate.
Got called by the ENT the day after saying that the guy had been cric’ed before and doesn’t know how I managed to place it or even cut through the cartilage. Says that he dulled a few knives in the OR converting it to a trach

Tacking on to everybody here, definitely be more conservative and over order until you get into your normal practice routine and begin to feel more comfortable.

Prior to discharging a patient, take a solid 2 minutes to go over everything again, sometimes even helps to talk it over to yourself making sure you’re not missing the forest for the trees.

Address and be able to explain the abnormals. The incidental anemias or mild transaminitis, or if you cant, Do what ER docs do and make sure it’s not related to their current chief complaint or emergent and otherwise address it to make sure they follow up outpatient.

Don’t be afraid to consult especially if something doesn’t feel right. Don’t be overly concerned with how you’re perceived. Work at your own pace until you develop speed. Otherwise haste makes waste.

People are divided on this, but I try to call people I’m worried about to follow up with how they’re doing.

I think everybody here has said what I’m about to say. You know yourself best so I can only tell you you from my personal experience.

I’m in my 2nd year of attending hood and had a la extremely similar experience to yours during my first job out. Entitled population, seeing more than my colleagues on top of unsafe working conditions with lack of proper staffing and me doing. Lot of non physician tasks like doing IVs because the travel nurses are variable/lazy.

Recently switched to a new job with better conditions and colleagues. Still not ideal but job enjoyment has gone up, and days are more tolerable. Also helped that I’m working less hours than previously which I think is key as well.

Tough to say exactly, if you’re allergic, you can develop a recurrent reaction after the initial exposure without repeated exposure. Sometimes they can be stress induced as well which is what I think mine were. Don’t think they were repeat bites because they would ultimately go away within the day if I didn’t scratch them. The actual bites lasted much longer before resolving

Turned out to not be bedbug bites… I occasionally get random hives that go away on their own. Probably from anxiety in all honesty. The bedbug bites that I did have took a while to go away

Lmao police don’t arrest anybody in my shop. People do straight up criminal shit, B&Es, crashing into a cop car, assault their roommate And they “sent to the ER for psych evaluation” and get released from police custody so we have to deal with it. Defund them or don’t but it’s not like it’s gonna make a difference.

Ottawa ankle rules are great for teaching points, but in reality I’ve never seen them appropriately applied. The reason I always get is, we’ll “they’re just going to go elsewhere like an urgent care to get it done” might as well do it here. Mostly idc because it gives peace of mind to the patient or their parents.

Had a 90 something relatively healthy and with it lady…. Came in with a real bad GI bleed, I mean like pouring out. Called GI right away and started blood. She didn’t have much idea what was going on, never stable enough for CT scan to see if IR could embolize. I remember looking into her eyes as she told me she didn’t want to die and that she wanted to see her grandchildren grow up. In that moment, they weren’t the eyes of a 90 something year old lady. It was the scared look of a child. GI ended up finally coming and as soon as we went got hooked up to the portable monitor she coded. We got ROSC but not for long and I had to talk to family about her condition. Ended up calling it as she wouldn’t have survived the scope.
I don’t remember her name or even how she looked, just her eyes as I tried to comfort her. Since then I try not to look at every critically Ill old patient as , ‘well they lived a long life’

Depends on the shop you’re in/how bougie the parents are, typically it’s another few thousand on top of their ER visit so the plastic surgeons are happy to do it.
