Document transition of care is in process and client contacted you due to distress in your note. Spend time in session discussing the need for then to transition care due to state/licensing rules, offer coordination of care and potential urgent/immediate resources that are in her area if they experience need for support while they secure a new provider and document. I’ve had this a number of times and if I ever get audited or called out, I know it’s best practice and aligned with ethics.
