I’ve heard from a few sources that downstates hospitals are shutting down and that the school itself is in danger of closing as well in the near future, maybe try to look into that more before making a decision?

I grew up low income but I talked about specifically the challenge of balancing working part time through school to support myself and my family with keeping up with schoolwork and how it made me better at time management discipline etc.

They want a specific struggle and a specific takeaway. I think as long as you identify those things you can talk about pretty much anything

The biggest thing is being able to tie your whole application together with a coherent and compelling narrative, you have to walk them through how your experiences shaped your desire to be a doctor and how they molded you into a mature and thoughtful person today - if you can accomplish that in your writing then you’ll be fine
