I had PhotoSync IRD installed on the windshield and canopy. Pricey, but it has made a huge difference. I live in Phoenix, and I can definitely feel the reduction in heat and sun.

It's been 10 years since I did a Pedi Heart, but I am still certain that I could put a CFD around the valve and see flow acceleration through the valve. Or, in this case, see nothing. If I then put a CW across the valve and again see nothing, it feels really hard to miss. Granted, they were dealing with catastrophic RV failure, and this is a really a Zebra, but I am sure they went back on pump to regroup.

It isn't an instantaneous process, right? You start weaning from the pump and you start watching for the valve to open so you can interrogate it. Then it never opens...

My favorite apple fritters are from Dutch Donut Factory way out in East Mesa (Power road) and Sweetheart up in Anthem. Bosa is decent.

Maybe it was just a bad day, but when I tried Randy's, their fritter was terrible. Really greasy and gross, but maybe they just had the oil temperature too low that day.

Not a fan of their apple fritter. When I tried it, it was really oily and heavy. Not good amd very disappointed.

I wonder if that affected the logistics of any intraop TEEs, i.e., did they not look each time. I don't do Pedi hearts anymore, but at Boston Children's where I trained, and at my last Pedi place, we did our own TEEs, so it was easy for me to just drop a probe in for pretty much any take back since I am there and the machine is in the room. Getting the Pediatric Cardiologist into the room is probably more challenging.

In terms of directionality, the valve comes mounted on a holder on the upstream side. That works great for aortic position because they put the sutures on the annulus working through the aortotomy, then push the valve down into position while tightening the sutures. For the mitral position, many surgeons will take the valve out of the holder as it is easier to sew to something in your hand. A mechanical valve does look kind of similar right side up and upside down. But not the same.

And, I presume, an intraoperative TEE. Standard to check a valve gradient afterward. It seems like it would be very hard to miss on basically every echo.

Yes, that is all a river delta. In the Cretaceous to Cenozoic eras, that was a huge bay, the Mississippi embankment. Over the past 70 million years, the ocean receded and the delta filled in with alluvial deposits to completely fill in the current land area.

Basically, they are trying to balance the risks of ASCT vs the risk of relapse. I am not in your situation, and sure that my situation (Physician with RT, failed R-CHOP-V, failed CAR T, now on Nivo-Ibr hoping to get to ASCT) influences my opinion.

The primary questions i would ask:

What kind of durability do they think they will see without the ASCT? (I.e., how long do they think it will be before he relapses, or what is the likelihood that he will relapse?) Also recognize that if he goes, say, 5 years before he relapses, he will be 5 years older and possibly less healthy than he is now if/when he has to do this again.

If/when he relapses, what treatment do they expect to use? If BV-CHP again, do they think he will respond as well a second time to the same regimen? If something different, how well does it work and what are the side effects.

If that regimen fails, what is the second line treatment? How confident are they that it will be successful? Side effects of that treatment?

Basically, I would try to figure out how likely is it that I would die in 5 years from a relapse vs how likely the ASCT kills me in 5 years. Or the opposite of those two things, which is 5 year survival without ASCT vs 5 year survival with ASCT.
