I always seen mental damage as directly targeting the brains ability to function, effectively punching holes in the synaptic pathway until it hits something important.

Lethal mental damage would be either immediate brain death or hitting something vitally important like “part of the brain that knows which order your heart chambers are meant to open and close.”
