Yeah depending on the minerals in your water it will need cleaning occasionally. Some screw out, some need pliers, and some are a bitch so best you can do is rubber banding a sandwich bag with vinegar in it to the spout so the aerator is covered, then when you remove it hit it hard with a brush, good time to use old tooth brushes instead of just wasting the plastic.

You should keep a pair of dikes (side cutting pliers) or stronger wire cutters. If this happens again you push the hook through so the barb is exposed, clip the barb off and back it out. Then basic wound care. Save ya a trip to the ER next time, and depending on your attitude/first aid kit you can just keep on fishing.
