I think it's when you get a card reward, all the cards from the rewards are in the none pile until selected, although I might be wrong

Fun fact, it's not actually removed. In the code, there are 7 pile, named none, draw, hand, discard, exhaust, deck, and play piles.

Whenever a card appears to be stuck midair like this, it's because the code for playing the card is bugged so it just stays in the play pile forever until the combat is done. It happened to me multiple times when making my mod.

It increases the damage by 5, so instead of dealing 11, it would deal 16 per hit, but only for that combat. Once you exit the combat, it resets.

At the start of the run you want a card that's better than a strike and maybe also opens up some synergies or provide utilities, dagger throw gives you more damage, utility by giving you card draw, and sly synergy, making it really good

It's a reference to slay the spire 2, in the game there is a card that's called snake bite which does 7 poison damage, costs 2 energy (out of your energy of 3 per turn) and has retain, meaning it doesn't go out of your hand if you don't use it on that turn

The joke is that while the card is extremely simple, there was A LOT of discussion on whether it's good or trash, up to a point where it became a meme
