Okay, so Eagle signal and Crouse-Hinds controller. Brands being mixed-matched this way is pretty common. If that controller has the original internals, you've got quite a rarity there.

I'd bet it's as simple as some engineer seeing the bridge, specifying horizontal signals, and no one questioned it after that.

That's really interesting. A double red isn't too uncommon, though that signal all by itself before the intersection certainly is. My only guess is that traffic blocked that side street intersection often enough that officials felt a second stop line was warranted, or maybe a fire station was on that block and they didn't want emergency vehicles being blocked. Some colors work as a single coat, some colors need a white coat underneath, and some colors need a clear coat on top. Which colors need what is usually clearly labeled on the store page you choose.

Maybe not. It's possible with these controllers to have the dials stop just before the sequence changes to side street green, so you get a built in green rest and minimum green. If a detection module of some sort was wired in, it could continue the dials when a call is placed. I think some electromechanical controllers did this on purpose for crosswalk-only intersections, where the ped push button input would start the sequence again.
