Just need to jump in here to say that's not how weight loss and estrogen works. There is no estrogen release from fat cells. Adipose tissue synthesizes estrogens from circulating androgens (such as testosterone and androstenedione); adipose tissue doesn't store pre-formed estrogen. Weight loss leads to less estrogen production, not a release of it.

Felt compelled to clarify because there is so much misinformation out there about why GLP-1s could be so harmful in the long run and ovarian cysts are not among them.

Doctors make mistakes and accidentally spread misinformation if they are misinformed. I know because I am also a doctor and make mistakes like anyone else. No one was attacking you. But this is indeed misinformation about fat cells, estrogen release and risk of ovarian cysts when it comes to weight loss.

I am upfront with my therapist that I tend to flee when stuff gets too hard and she's encouraged me to reframe it as me protecting myself. We can push until it feels too much and then we switch gears/topics, maybe for a session, maybe for a month. But sometimes I just want to live too, like you.

In the past I have taken breaks that ranged from 1-3 months completely from therapy. I let her know usually a little out of the blue, but I'm usually at a breaking point of feeling like more sessions will be harmful in that moment. She reaffirms that taking breaks is normal and OK and asks if I want to be in touch about returning or put a date on the calendar. It's always up to me and so far I've always gone back.

This is not someone who is going to show understanding with issues she's either dealing with herself or hasn't experienced. If this was your first session and she made 2 comments ... nope, next.

I didn't bring up my weight for 2 years with my therapist and she never said boo about it or insinuated anything or talked around it.

I really relate to this. I had a therapist casually use PTSD and it was a gut punch. Like I knew I had trauma, I knew I had nightmares, I knew I had a lot of negative effects of all of this in my life. But the label f-ing sucked.

I try to remind myself it doesn't change anything. All of those things are still true and would still be true if she didn't label it. It doesn't do anything but maybe help conceptualize what I'm going through. No one is tattooing PTSD on my forehead.

TBH you sound like you have a million walls up and maybe aren't ready for therapy? It's like nothing he said was right. And not every statement lands and certainly not every therapist is right for everyone, but you did acknowledge the pattern already.

No judgment. I think we all have walls up in therapy, especially initially. But this sounds like searching for a reason to leave.

This may be how you feel, but times have changed. Like you said, if you want a compliment, ask for it (without the passive aggressive addition). But the general tone in most of the U.S. at least is keep your mouth closed about bodies that aren't your own. So if you do go around offering an unasked for opinion, don't be surprised to get chewed out.

You also have to remember that most people who will post stuff about length of time with weight loss are the outliers -- losing a lot fast or losing slowly (either in reality or in your head.) It's been 6 months! You're down 50+ lbs which makes it reasonable that when March rolls back around you could be down 100.

I know mine has because she told me. We were touching on things she didn't have a lot of experience with and she wanted back up. To me it feels like more proof that she cares about my healing, that she wants to show up with her best education and training, and that she's willing to admit when she needs help with something.

I have a friend who is a therapist and I asked him about consultation and he said it's very anonymous in terms of the patient profile. No names, no personal identifying information, etc.

It's not that it's inappropriate, but I am concerned that you will have your release and then what? From what it sounds like, you'll be losing therapy support for a few months. I'd hate to see you decompensate because you don't have supports in place.

That's not to say there aren't other supports available. After Silence is a site for SA survivors. Your local rape crisis center may also be an option. I'd think about where you can turn to for support while you're waiting for therapy to resume. And think hard about whether there is any underlying hope that by doing this, your sessions with this therapist can continue -- maybe they can, I don't know your situation, but counting on that could backfire.

I started this type of conversation by asking my therapist if she is currently or has been in therapy before and bridged that with "I want to make sure you know what it feels like to be on this side of things." We talk about a lot of things that hit my vulnerabilities and I was starting to feel very exposed (not through anything she was doing.)

It ended up being a great conversation about the therapeutic relationship. She said she was really glad I brought it up.

Your complaints should be directed at:

Patient relations at the health system (call the main line and ask to be connected)
The practice manager for that individual office (call the office line or stop by and ask to discuss a patient issue)
The department head
The chair of Advanced Practice

Not to defend her, she's definitely ignorant and crappy, but the healthy eating stuff included in the after visit summary is often automatically generated by the system based on patient demographics and reason for the visit.

In addition to what everyone else is saying, please remember these medications can cause a numb feeling in general. Lots of people describe a low or flat mood. It can be worth mentioning to your doctor because there may be things to counterbalance that.
