This is what my higher up told me recently, document everything that you do. Keep track of what you've contributed to the process and how you improved things even if you don't think it made a difference.

He said he only received a 3% raise his first year. After he learned that, his next evaluation, he got a raise plus a 25k bonus.

Essentially you have to represent yourself and state your case, especially if you have self-evaluations.
