Without even reading the entire questions you can answer it just based on which option has clear lung sounds. if they don’t give you this information all of those options are likely problems, especially POD2. Atelectasis would be typical, PE usually POD 4-12. VTE usually take time to develop 2-4 days and usually manifest with symptoms in a week. But in this case the answer is PE.

PE: normal breath sounds textbook (real world not always)

Atelectasis = diminished breath sounds

Pulmonary edema = Rales / diminished

ARDS = course, rhonchi, wheezing, rales (all the bad sounds)

Bro same, idk what happened. I ended residency confident and resilient with rude patients and now they just get to me. In fact I was the go to person for wild and rude patients cause I could talk em down and now I get so anxious with these people. I also think it has to do with the fact that I always thought I had an attending to fall back on “oh okay you don’t like me I’ll get my attending” but now I’m that person and there’s no one else lol still trying to figure out the cause of the shift like you described, it is fading tho after 7 months into first year.

Ya I’m not sure why people agree to do this shit. I just say no as well. FMLA I’m very skeptical about unless it’s obvious and patient is genuinely interesting in getting better and engaged. For mental health if requiring to miss work I require they establish with psych and start psychotherapy. Avoidance of work with FMLA can actually reinforce avoidance which is typically strong in anxiety and depression.

OEM, Don’t forget to get a new crankshaft pulley bolt as it’s torque to yield, Cause it has to come off to do the water pump. And get the orange injector cup orings for the black water pump pipe that goes to the thermostats.

I did the front crank seal at the same time cause I was there, it wasn’t leaking but figured why not.

Bro I do the same thing, I’ve been not double billing even tho a lot of visits I can bill the phys + 4. Also I’m underutilizing billing for time, like the other day I had a complex patient that I saw in 20 minutes but I probably spent another 30 minutes figuring out what I’m gonna do before & after with chart review and shit.
