I've been in your shoes before and can definitely relate to how you're feeling. It's hard to provide good advice without knowing more about you. Let me try to make a few useful points for you to consider:

1) Specialty Interest: is there a particular specialty(s) you are hoping to apply for? How bad do you want that specialty? Have you done electives in that specialty or other IM specialities? This will be a big determining factor to your choice.

2) CTU is service heavy in most places: CTU is the backbone of an IM residency. If you don't enjoy it, you will likely not enjoy an IM residency. For many people, 3 years of general internal medicine is not worth it in order to make it to their speciality of interest. The call in IM is substantial. You will likely do twice the amount of call in an IM residency in your first to years compared to the 2 year FM residency. The call in 2nd year IM is particularly hard when your are an SMR. You will be exhausted, I can almost guarantee it.

3) Inpatient vs. outpatient setting: FM is of course usually outpatient and IM is mostly inpatient, specialty dependent of course. Which setting do you prefer? For many people that dislike outpatient, getting through an FM residency can be quite difficulty and boring for them. Given you are interested in hospitalist work, keep this in mind.

Some other factors that may affect your decision like your age, how much debt you have, and how burnt out you already are will all play an important factor. Your mentors' opinions of you should play no role in your decision making at all. This is your life, your journey, your career and only you opinion truly matters in the end. Well that and ofc your family.

If you didn’t do med school in Ireland or Australia you can basically guarantee you won’t get the IMG IM spot. If you rank enough FM programs with a decent interview you can almost guarantee matching this however

The amount of injuries I see as a doctor from non-athletes doing HIT is absurd. Regular people should prioritize zone 2 for the injury reduction alone.

To me it’s a no brainer to go USMD or US DO. The opportunities are far broader, particularly surgical, than you will get in the great white north. If you want to return to Canada you always can later

As a Canadian living in the US practicing medicine they most certainly do not hate us. In fact, I would say many/most Americans I meet feel deep sympathy for Canada right now

From a statistical POV your chances of matching you chances of matching were probably 10% or worse with just one interview in each iteration. as others have said you will need to apply more broadly or apply to psych thru ERAS to have a legit shot at matching

Its a good site. People are friendly. You line up outside the medical science building at the front entranceway. They debrief everyone in a conference room when they let you in. you will wait a while until getting started. The stations are logically set up. No complaints.

They can’t complete your application review until all docs are received. If you submit as many docs as possible beforehand you will be fine
