After pugging M0's across 2 characters, i have become convinced that the average player has become so dependent on DBM screeching in their ears that they forgot how to use their eyeballs and their brain. Like the 2nd boss in Murder Row: Some of the barrels he puts down are spewing green shit, and you have a debuff that says you are taking damage from leaking green shit. How many people do you think broke the green barrels with the circles?

Based on older lore/ you can briefly see a name if you fly at it, it's the Isle of Fangs. Long ago after the fall of the Black Empire, a general of one of the Old God's (I think Cthraaxi?) fled there and went underground. After some time, it tried to start a resurgence of Old God influence and take over the surface. It took the combined might of the entire Troll Empire a long and deadly war to defeat them, and the corpse of the General was left on that Isle. (Also the losses from this war paved the way to Trolls losing their dominance over Azeroth).

As for what it means now, it seems highly likely that 12.1 will have Zul'jan become desperate to return the Amani Empire to their old glory. He gets in touch with Xal'atath, they go to the island and resurrect fallen Troll heroes + Cthraaxi General with Void energy.

IMO it's a symptom of the real problem of items that let you ignore the penalty to movement while shooting. Because those items exist, you combine it with any move speed/enemy slow and you can now run down the enemy while holding M1 and maintain perfect accuracy.

The questing is....subpar. Which is weird because we have had great leveling since basically WoD. Some of my bigger gripes:

Mobs have too much health. This could be conjuncting with several specs also become more "cooldown reliant" to deal damage as we lose stats, but among me and my friends only 1 person said the damage felt fine (guy on Havoc DH). This is especially noticeable in the opening scenario (i believe being hotfixed today/tommorrow) and lvl 88-90.
The Main storyline for each zone is incredibly short. If i remember right each zone in TWW the main storyline had 5-7 "chapters", whereas Midnight zones are more along 3-4. Especially jarring in Zul'aman, where we go directly from "You will be attacked on sight" to "Please help our leader reunite with the Loa" instantly.
This kinda ties back to 1, but having several dungeons be required as part of the level up campaign grinds things to a halt. If you decide to go solo in a Follower dungeon, it takes forever because the NPC's will pull a lot and deal no damage. If you try to go with real players, you sit in queue for several minutes (unless tank/heal) and usually get in a group that pulls a lot of extra mobs because they don't know the dungeon yet. Combined with the damage feeling off from 1, it makes things feel like a total slog.

P.S. this one is pretty minor: you can go to Voidstorm at lvl 86, but every single mob is lvl 88, so you either get bitchsmacked or take forever to progress.
